羟基磷灰石义眼座在眼整形术式中的应用与并发症的分析

摘　　要：目的 探讨羟基磷灰石（HA）义眼座置入的手术时机与整形手术结合的方法以及术后并发症的防治方法。方法 对86例患者以HA义眼座置入为主要步骤的眼部整形病例进行总结分析。按置入时机分为Ⅰ期置入和Ⅱ期置入，置入方法可分为Ⅰ～Ⅳ型，对这几种方式的例数、手术过程、特点予以统计及描述。重点讨论HA义眼座的优点、手术的注意点及并发症的特点。结果 86例HA义眼座置入患者，同期行结膜囊整形术20例，眼睑整形修复术8例，眶骨修复术2例。78例患者术后Ⅰ期愈合。义眼活动度及眼外观形态均较满意。6例术后结膜囊裂开，2例发生义眼座暴露脱出，4例术后出现眶内感染。结论 HA材料制作义眼座，具有较好的生物相容性，排斥反应低，置入后义眼活动度好。对改良HA义眼座的置入方法结合整形手术，能提高手术疗效，减少并发症的发生。Objective To explore the combined approaches of ocularplastic surgery with oneurrent HA orbital implantation and the methods of the prevention and cure of the postoperative complications. Methods The authors reviewed the data retrospectively from 86 cases who underwent the ocularplastic surgery with HA implanta- tion. Results The implantation methods were classified into four types. HA were implanted primarily or secondarily in 86 cases. Conjunctiva sac plastic was done simultaneously in 20 cases. There eyelid plastic surgery was done in 8 cases and orbital plastic surgery in 2 cases. The cosmetic Appearance and pseudo - ophthalmic mobility were excellent in 78 cases. Con）unctival wound dehiscence occurred in 6 cases, HA exposure in 3 cases and orbital infection in 4 cases. Conclusion The application of HA orbital implantation in ocular surgery is a good method with few complications and satisfactory cosmetic effect.
