孕妇血清绒毛膜促性腺激素联合甲胎蛋白产前筛查唐氏综合征的研究

摘　　要：【目的】探讨人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)联合甲胎蛋白(alpha-fetopro-tein,AFP)产前筛查唐氏综合征(Down’s syndrome,Downs)的临床应用价值。【方法】以HCG和AFP为指标,对840例孕周在14-21周的孕妇进行血清HCG和AFP检测,同时对胎儿/新生儿进行染色体检查。【结果】840例接受筛查的孕妇,32例筛查阳性,808例筛查阴性,在筛查阳性孕妇中发现Downs3例,筛查阴性孕妇中发现Downs1例,本方法的灵敏度、特异度、假阳性率、假阴性率、阳性预测值和阴性预测值分别为75.0%(3/4)、96.5%(807/836)、3.47%(29/836)、25.0%(1/4)、9.38%(3/32)、99.9%(807/808)。【结论】孕妇血清HCG联合AFP筛查唐氏综合征经济、有效,且操作简便,有临床推广应用价值。[Objective] The purpose of this study was to investigate the value of human chorionic gonadotropin （hCG） and alpha-fetoprotein（AFP） in screening for Down＇s syndrome. [Methods] 840 women underwent testing between 14^th and 21^nd week of pregnancy. The chromosome of fetal and newbown were analysed in the same time. [Results] 32 women were screen-positive and 808 women were screen-negative. 3 Down＇s of screening-positive women and 1 Down＇s of screening-negative women were confirmed by karyotyping. The detection sensitivity and specificity for Down＇s syndrome were 75.0%（3/4） and 96.5%（807/808）. False-positive rate and false-negative rate were 3.47%（29/836） and 25.0%（1/4）. The positive and negative predictive value were 9.38%（3/32） and 99.9% （807/808）. [Conclusion] The study showed that maternal serum hCG and AFP screening for fetal Down＇s syndrome was economical and simple and feasible in the clinical practice.
