摘　　要：目的：探讨经直肠彩色多普勒超声对前列腺癌的诊断价值。方法：应用经直肠彩色多普勒超声对65例临床拟诊为前列腺癌患者进行检查，其中病理证实46例为前列腺癌，分析研究此46例前列腺癌患者前列腺内异常结节回声情况、测量其大小，根据结节内血供情况丰富与否分为四级（0～Ⅲ级），测量其血流速度及阻力指数。结果：前列腺癌多发生于外腺（周围区、中央区），其前列腺癌结节回声复杂多样，血供增多为主，动脉流速15～30cm／s，RI≥0．75时，可提示前列腺癌。结论：经直肠彩色多普勒超声对前列腺内部结构显示清晰，且能检测到前列腺癌内微小血管的血流速度及RI值，虽然与其他前列腺疾病有重叠，但结合临床指诊及化验室PSA检查，可对前列腺癌做出早期初步诊断，从而提高了前列腺癌的诊断率和治愈率。Objective：To evaluate the value of transrectal color Doppler ultrasound in the diagnosis of prostate cancer（PC）. Methods： Sixty-five patients with PC were examined with transrectal color Doppler ultrasound, among which forty-six patients were confirmed. Analysis the abnormal echoes and the sizes of the forty-six patients, and the amount of blood supply were graded respectively on a scale of 0-3. The speed of blood and the resistance index （RI） were measured as well. Results：The PC often occurred in the outer gland （peripheral or central area）. The echoes of PCs nodes were complicated and varied, and the blood supply mostly increased. When the floe ratgof artery was among 15- 30cm/s and RI≥0. 75, the PC should be highly suspected. Conclusions： Transrectal color Doppler ultrasound could not only clear the inner structure of prostate, but also detect the flow rate and RI of the tiny blood vessels in prostate. Though the numerical values of flow rate and RI may overlap with those of other illness of prostate, combined digital rectal examination and PSA of laboratory could make the early and tentative diagnosis of PC to increase the rate of treatment and cure of PC.
