“中立位有限活动”治疗颈椎病

摘　　要：目的:总结"中立位有限活动"治疗颈椎病的疗效。方法:对357名颈椎病患者施行"中立位有限活动"。即患者颈部处于某一位置时没有颈椎四肢酸痛麻木等症状,此为中立住。以此为基准,主动限制各方向的活动范围,一般来说伸在45°范围内,旋转不超过30°,各方向活动轻柔和缓,避免较长时间处于某一位置。间歇性进行控制下颈部肌肉的等长收缩活动,以此促进组织的血液循环和调节椎间盘内应力负荷。结果:经过平均38月的随访(范围12- 60月),优良率为89.07%。有随访X片的87例患者,其颈椎X片无明显变化。对比32例患者治疗前后的MRI,29/32例无明显变化。3/32例椎间盘突出程度加重,症状加重,予以手术治疗,恢复良好。结论:"中立位有限活动"对颈椎病患者有良好的治疗作用。Objective： To assess the clinical effect of limied motion around neutral position＂ on the cervical spondylisis. Method： For ＇limited motion around neutral position＇, it is named neutral position on which the head is maintained without the presence of pain and/or numbness on neck and extremites.The motion at all directions is limited actively around the neutral position. Generafly speaking, the range of motion for neck extension and flexion is within 45° and less than 30° for neck rotattion The motion is conducted softly and smoothly. It is avoided that the neck be maintained at one position for a long time. Intermittent isometric contraction of cervical muscle is employed to improve blood drculation in the soft tissue of cervical spine and regulate the intradiscal stress. We call the regime described above ＇ limited motion around neutral position＇ . It is introduced and instructed for 357 patients with cervical spondylosis.Results： The excellent and good results rate was 89.07% at an average follow - up period of 38 months （range： 12 -60 months） .X- ray film analysis of 87 cases showed that the^e was no significant change in the x - ray films raked before the treatment started and at the last follow - up. For MRI analysis of 32 cases, there were no sig- nificant changes in 29 cases, for the rest 3 cases, it was found that symptoms deteriorated with larger prolapsed disc in cervical canal.They were operated on with a good result.Conclusion： A satisfied clinical result is achieved with a regime called ＇limited notion around neutral position＇ on cervical spondylosis.
