角膜上皮移植术治疗陈旧性眼部烧伤的临床应用

摘　　要：应用角膜上皮移植术联合干燥板层角膜移植术（KEP－DLKP）治疗16例16眼曾行新鲜供眼材料全板层角膜移植术（LKP-F）而失败的陈旧性眼部烧伤患者，术后随访3～12（平均8.96）个月；同时，与新鲜供眼材料的板层角膜移植术术后同期结果相比较。KEP-DLKP和LKP－F术后：角膜植片透明率分别为87.5％和31．25％．P＜0．005：角膜新生血管复发率分别为6．25％和81．25％，P＜0．005；持续性角膜上皮缺损发生率分别为0％和87．5％，P＜0．005；移植排斥反应发生率分别为12.5％和6.25％．P＞0．05；手术前后盲目率前者下降有显著性（P＜0．05），后者无显著改变（P＞0．5）。据此．本文提出KEP－DLKP可用于治疗陈旧性限部烧伤改良基地的全板层角膜移植术后失败者的治疗。To investigate the surgical treatment for the graft faiture after lamellar keratoplasty on the old burn eyes.The combinated surgury of keratoepithelioplasty and lamellar keratoplasty (KEP - DLKP ) was performed for 16 cases (16)eyes on which lamellar keratoplasty from fresh donor (LKP - F) was per formed formerly.Followed up for 3 to 12 (means 8. 96 ) months,the results of KEP - DLKP and LKP - F were compared. In KEP - LKP and LKP-F,the graft transparent rate was 87. 5% and 31. 25% (P <0. 005 ) -the recurrence rate of the cornea1 neovasculerization was 6. 25 X and 81.25% (P <0. 005), the occurence rate of the persistant corneal epithelial defects was oN and 87. 5 X (P<0.005 ), the occurence rate of the corncal graft rejection was 12. 5% and 6. 25% (P >0.05 ). In KEP-DLKP.the vision acuity was increased postoperatively, but there were no change in LKP - F. The rcsults suggested that KEP-DLKP is suitable for treatn1ent of the graft faiture after lamellar keratoplasty for improving the basement of the old burn eyes.
