摘　　要：目的评价１３１Ｉ＿ＭＧ７放射免疫显像对胃癌患者的临床应用效果。方法病理确诊的胃癌１０例，贲门鳞癌１例，于静注１３１Ｉ＿ＭＧ７２４，４８，７２ｈ后应用ＳＰＥＣＴ进行放射免疫显像，观察对胃癌及转移病变的定性、定位诊断效果，并与手术及内镜所见对比。结果１１例中放射免疫显像清晰者有１０例，阳性率９０９％，其中９例为胃腺癌，１例为贲门食管鳞癌。Ｔ／ＮＴ平均为２７０。肿瘤显像的部位、大小与手术或内镜所见相等，转移灶也显影。大小与手术或内镜所见相符，转移灶也显影。１例胃腺癌切除标本，在１３１Ｉ＿ＭＧ７用后９６ｈ，组织Ｔ／ＮＴ仍达２１７，１３０ｈ后胃标本放射免疫显像，肿瘤部位仍显像清楚。结论１３１Ｉ＿ＭＧ７ＲＩＩ是诊断胃癌特异性定性定位方法。食管鳞癌显像表示鳞癌细胞表面也有ＭＧ７抗原表达，值得进一步观察。AIMS To evaluate the clinical effects of radioimmunoimaging (RII) by using 131 I labeled anti_human gastric carcinoma mAb MG 7 ( 131 I_MG 7) in patients with gastric carcinoma. METHODS In 11 patients with stomach and cardia neoplasms, 24, 48 and 72 hours after 131 I_MG 7 injection, RII were carried out with SPECT, and the diagnostic effects of tumor imagings for gastric carcinoma and its metastatic lesions were oberved. The results were compared with the findings of surgical operations and gastroscopies. RESULTS RII were positively visualized clearly in 10 patients (90 9%), with 9 patients with gastric adenocarcinoma and one with squamous cell carcinoma of the cardia orifice, T/NT ratio was 2 70 in average. The scintigraphic tumor positions and sizes in the stomach were similar to findings by operations and gastroscopies, and the metastatic lesions were also detected. The tissue T/NT of one resected stomach with adenocarcinoma was still 2 17 after 96 hours of 131 I_MG 7 injection and tumor imaging was still visualized clearly in tumor region at 130 hours after 131 I_MG 7 was given. CONCLUSIONS 131 I_MG 7 RII is a specific and satisfactory diagnostic approach in determination and localization of gastric adenocarcinoma and its metatatic losions, and 131 I_MG 8 is also positively visualized in one patient with squamous cell carcinoma indicated that there is expression of MG 7 antigen.
