摘  要:目的提高脑单发转移瘤与星形细胞瘤的鉴别诊断水平。资料与方法将42例单发脑肿瘤分为脑转移瘤组和星形细胞瘤组,对其性别、年龄及MRI特征,包括肿瘤最大直径、边界、瘤周水肿、占位效应、出血、坏死与囊变及强化方式,进行回顾性分析总结。分析各因素间关系时采用χ2检验,等级资料采用两独立样本非参数检验(Mann-Whitney U)。结果脑转移瘤组与星形细胞瘤组的性别、边界、占位效应、出血差异无统计学意义,年龄接近显著差异,肿瘤最大直径、瘤周水肿、坏死囊变及强化方式差异有统计学意义。结论肿瘤最大直径、瘤周水肿、坏死囊变及强化方式等MRI表现特征对两者的鉴别诊断有重要意义。Objective To improve the level of differential diagnosis of the single brain metastases from astrocytoma. Materials and Methods 42 cases of single brain tumor were divided into two groups of metastases and astrocytoma, and the sex, age and MRI features of each tumor were analyzed. The MRI features included tumor maximal diameter, border sharpness, surrounding edema, effect of occupying, hemorrhage, necrosis and cysts, contrast enhancement. The statistical analysis was dealt with the X^2 test and the grading data were done by Mann Whitney U. Results The sex, border sharpness, effect of occupying and hemorrhage between the single metastases and astrocytoma showed no significant difference, and the age between the two groups showed close to significant difference, while the maximal diameter, surrounding edema, necrosis, cyst and contrast enhancement showed significant difference. Conclusion The MRI features including maximal diameter, surrounding edema, necrosis, cyst and contrast enhancement are useful and important for distinguishing the single brain metastases from astrocytoma.
