摘  要:目的利用二维化学位移成像(2DCSI)检测帕金森病(PD)的基底神经节代谢物浓度。探讨PD的基底神经节代谢物浓度是否变化。评估2DCSI技术检测PD的价值。资料与方法符合临床诊断标准的PD患者26例。从健康志愿者中抽取与PD组年龄、性别相匹配的26名健康者作为正常对照组。采用2DCSI技术检测所有入组对象的双侧豆状核、尾状核的代谢物浓度,包括氮-乙酰天门冬氨酸/肌酸复合物(NAA/Cr),氮-乙酰天门冬氨酸/胆碱复合物(NAA/Cho)以及Cho/Cr比值。比较PD组和正常对照组的基底神经节相对应核团的代谢物浓度之间的差异。结果PD组双侧豆状核的NAA/Cho和右尾状核的NAA/Cr、左侧尾状核的Cho/Cr比值明显低于正常对照组(P<0.05)。左侧豆状核Cho/Cr比值明显高于正常对照组(P<0.05)。结论PD患者的基底神经节代谢物浓度与正常人有差异。2DCSI作为一种可用于检测基底神经节代谢物浓度的无创伤性的方法,将成为检测正常脑的生理性和病变脑的病理性代谢物浓度的变化,尤其是神经元变性性疾病的一种重要手段。Objective To discuss the levels of N acetylaspartate (NAA), choline (Cho), and cretine (Cr) in the basal ganglia of ,PD patients using 2D chemical shift imaging (2D CSI), Materials and Methods 26 patients with Parkinson' s disease and 26 volunteers as control were examined by 2D CSI. 2D CSI imaging acquisition was performed in bilateral caudate, lentiform. Voxels were selected in basal ganglia area, and 1 H-MRS were processed to determined the metabolite ratios, including NAA/Cho, NAA/Cr, Cho/Cr, Statistical analyses were performed using SPSS 11.5 software. Results Significant reduction of ratios of NAA/Cho, NAA/Cr and Cho/Cr and significant increase of ratio of Cho/Cr were observed respectively in the bilateral lentiform, right caudate, left caudate and left lentiform of PD patients compared with controls ( P 〈 0.05 ). Conclusion 1 H-MRS can detect the abnormal metabolite changes of Parkinson' s disease, which may play an important role in clinical studies and applications for various conditions of metabolic disorders of the human brain.
