摘 要:目的研究健康成年人空腹胃肠动力及血浆胃动素浓度的变化,以探讨2者的关系。方法采用灌注式测压方法,检测15例健康志愿者空腹胃肠动力,并根据压力波形特点分期抽血采用放免法检测血浆胃动素浓度。结果所有志愿者空腹胃肠运动都表现为反复发生的运动复合波(migrating motor complex,MMC),MMC周期平均持续106±32 min,根据压力波形特点MMC可分为3期。Ⅰ期平均持续38.9±22.4 min;Ⅱ期平均持续62.3±20.1 min;胃窦Ⅲ期持续2.8±1.2 min,十二指肠Ⅲ期持续4.5±1.8 min。MMC各期的血浆胃动素浓度不相等(P<0.05),Ⅱ期和Ⅲ期的胃动素浓度均较Ⅰ期高(P<0.05),Ⅱ期和Ⅲ期血浆胃动素浓度相比较无统计学差异(P>0.05)。结论健康成年人血浆胃动素浓度随MMC周期波动,胃动素可能参与MMC的发生。Objective The aim of this study was to describe the interdigestive gastroduodenal motility and detect plasma motilin to determine whether motilin was responsible for the motility observed in healthy adult subjects. Methods The interdigestive gastroduodenal motility was detected by means of peffused gastrointestinal motility manometer in fifteen healthy subjects. Plasma motilin was detected according to the characters of contractions in different phase by the radioimmunoaasay method. Results All healthy subjects presented a cyclic pattern of fasting motor activity with a mean duration of 106 ± 32 rain( migrating motor complex, MMC ). Phase I of MMC lasted 38.9± 22. 4 min. Phase Ⅱ lasted 62. 3 ± 20. 1 rain. Antral phase Ⅲ lasted 2. 8 ± 1.2 min, duodenal phase Ⅲ lasted 4. 5 ± 1.8 rain. Plasma motilin concentrations were different among the three phases of MMC( P 〈 0. 05 ). Plasma motilin concentrations in phase Ⅲ were not significantly different from phase Ⅱ ( P 〉0. 05 ), but both phase Ⅱ and phase Ⅲ were significantly higher than phase Ⅰ( P 〈 0. 05). Conclusion Plasma motilin concentration fluctuates in phase with the MMC. Motilin may be involved in the mechanism of MMC.
