摘 要:目的探讨神经生长因子对神经源性痛大鼠脊髓的保护作用及其机制。方法取成年雄性Wistar大鼠66只,随机分为3组,神经生长因子组30只,结扎坐骨神经后腹腔注射神经生长因子,20μg.kg-1;0.9%氯化钠溶液组30只,结扎左侧坐骨神经中段后腹腔注射等量0.9%氯化钠溶液;假手术组6只,手术仅暴露而不结扎坐骨神经。于术前和术后6 h、1,3,7,14 d分别进行行为学测定,记录大鼠缩爪与尾弹时间。应用免疫组织化学方法和图像分析系统检测脊髓中TNF-α和IL-6的表达,采用TUNEL法观察脊髓神经元凋亡,采用HE染色及尼氏染色法观察脊髓的病理形态学变化。结果与0.9%氯化钠溶液组比较,神经生长因子组大鼠脊髓TNF-α和IL-6表达降低,脊髓神经元凋亡指数降低且疼痛减轻(均P<0.01)。病理学检查显示,神经生长因子组脊髓组织损伤程度较0.9%氯化钠溶液组明显减轻。结论神经生长因子对大鼠脊髓有保护作用。其机制可能与抑制促炎性细胞因子的表达,从而抑制神经元凋亡和疼痛有关。Objective To probe into the protective effects of murine nerve growth factor(NGF) on the spinal cord in the rat with a model of neuropathic pain. Methods Sixty - six adult male Wistar rats were randomly divided into 3 groups : ① NGF group(n =30) , ② saline group (n =30) and ③ sham operation group (n =6). A model of neuropathic pain was set up in each of the rats of group ① and group ② by ligating the left sciatic nerve under chloral hydrate anesthesia. The sciatic nerve of each of the rats of group ③ was exposed but not ligated. The rats of group ① and group ② were given each an intraperitoneal injection of 20μg·kg^-1 of murine NGF and an equivalent amount of 0. 9% sodium chloride solution after the operation, respectively. Behavioural testings including the times of paw withdrawal and tail flicking in animals of all 3 groups were carried out 1 day before the operation, as well as 6 hour, 1, 3, 7 and 14 days after the surgery. Expressions of TNF-α and IL-6 in the spinal cord were assayed with the immunohistochemical methods and image analysis. Apoptosis of the spinal cord neurons was detected with the TUNEL method and the histopathological changes of the spinal cord were examined with light microcopy under HE and Nissl's stainings . Results The 50% paw withdrawal threshold in rats of the NGF group was significantly higher than that in rats of the saline group from the 3rd day after the operation to the termination of the experiment ( P 〈0. 01 ). From the 1st day after the operation to the termination of the experiment, the hot water flick tail threshold was strikingly higher in rats of the NGF group than that in those of the saline group( P 〈 0. 05, P 〈 0. 01 ). The expression of TNF-α and IL-6 in the spinal cord as well as the spinal cord neuronal apoptotic index in rats of the NGF group were significantly lower than those in rats of the saline group(P 〈0. 01 ).
