国产奥沙利铂联合亚叶酸钙、5-氟尿嘧啶治疗晚期胃癌的临床观察

摘　　要：目的观察国产奥沙利铂(L-OHP)联合亚叶酸钙(LV)和5-氟尿嘧啶(5-Fu)持续44小时静滴(双周疗法)治疗晚期胃癌的疗效和毒副反应。方法对39例晚期胃癌患者采用以奥沙利铂为主的方案进行化疗,评价疗效。结果39例患者,其中初治者25例,复治者14例;获得总有效率为48.7%。初治(一线)有效率为60.0%,复治(二线)有效率(RR)为28.6%。毒副反应主要为:白细胞减少发生率69.2%,外周神经感觉异常发生率为58.9%。结论L-OHP联合LV和5-FU持续静滴方案治疗晚期胃癌临床疗效较好。Objective To evaluate the effects and toxicity of Oxaliplatin （ L - OHP） combined with leucovorin （ LV） and 5 - fluorouracil （5- Fu） regimen continuous infusion 44 hours （therapy of double cycles） in chemotherapy for patients with advanced gastric cancer. Methods thirty - nine patients with advanced gastric cancer were received. All patients finished 3 - 4 cycles （two weeks per cycle）. Results Thirty - nine patients were valuable for clinical response. Thirty - nine patients achieved 48.7 % respectively. The response rate for chemotherapy najve （first - line ） and pretreated （ second - line） patients were 60.0% （ 15/25 ） and 28.6% （4/14）. The main toxicity were leneopenia （69.2%） ,with grade 3 toxicity was 12.8%, vomiting （41.0%）. The obvious side effect was neuron- sensory （58.9%）, which was caused by L - OHP. Conclusion Oxaliplatin combined with LV/5 - FU dripping of vein lasting chemotherapy regimen is high response and well tolerable for advanced gastric cancer, it is deserved to be spreader and applicator.
