老年人晚期贲门癌综合治疗疗效观察（附65岁以上老年患者38例分析）

摘　　要：目的对６５岁以上老年人晚期贲门癌３８例进行放疗加化疗综合治疗，探讨其疗效。方法患者均经病理或细胞学证实，均为Ⅲ、Ⅳ期病变，用６０Ｃｏ－γ线或直线加速器１０ｍＶＸ线前后野对穿照射，ＤＴ６５～７０Ｇｙ／６～７周放疗，同时加用ＵＭ（优福定及丝裂霉素）方案化疗。结果完全缓解１２例，占３１％；部分缓解２３例，占６１％；无缓解３例，占８％；总有效率９２％。１、２、３年生存率分别为４７％、３７％及１６％。结论对６５岁以上老年人不宜手术和失去手术机会的晚期贲门癌放疗加化疗的综合治疗是一种积极有效的治疗方法。Objective To observe the effect of combined radiotherapy and chemotherapy on aged patients with inoperable late cancer of gastric cardia. Methods The clinical data of 38 aged patients with late cancer of gastric cardia were reviewed. All the cases were confirmed pathologically. The patients were treated with combined radiotherapy and chemotherapy and classified as grade Ⅲ and Ⅳ.They were treated with either￣(60) Co γ-ray or 10 m V linear accelerator X-ray ; penetrating antero-posteriorly. The treatment lasted 6-7 weeks and the total tissue dose was 65-75 Gy. Chemotherapy regimen of UFT and mitomycin was used along with the radiotherapy. Results The number of patients with complete , partial and no remission was 12(32%) , 23(61%) and 3(8%) respectively. The overall effective rate was 92% and the 1- , 2- and 3- year survival rate was 47% (18/38) , 37% (14/38) and 16(6/38) respectively. Conclusions For aged patients with inoperable late cancer of gastric cardia,combined radiotherapy and chemotherapy might be a positive and effective measure.
