组织端粒酶活性和细胞调亡指数的测定在胃癌早期临床诊断的意义

摘　　要：目的:探讨胃癌组织中端粒酶活性和细胞凋亡指数与正常组织相比较在癌组织中的变化,及其在胃癌早期诊断和临床分期方面的意义。方法:采用端粒扩增法(TRAP)检测端粒酶活性,应用免疫组织化学方法及TUNEL法检测胃癌组织的细胞凋亡指数。结果:胃粘膜活检标本中胃癌组织端粒酶阳性率达88.1%相近,提示大部分胃癌组织端粒酶处于活化状态。此结果在胃癌的早期诊断中具有重要的意义。Objective： To study the relationship between telomerase activity and apoptosis index in gastric cancer contrast to normal gastric tissues, and to study their values in early diagnosis and clinical staging in gastric cancer. Methods.Telomerase activity levels were measured by a modified telomeric repeat amplification protocol （TRAP）. The measurement of terminal deoxnuceotidy transferase mediated d-UTP biotin nick end labeling （TUNEL） and the streptavidin peroxidase immunohistochemistry were used. The relationship between telomerase activity and apoptosis index in gastric cancer was analyzed. Results：The positive rate of telomerase activity reached 88.1% in the gastric cancer tissue but only 10% in the gastric tissue （P 〈 0.01） . The apoptosis index was 8.94±2.15 in early gastric cancer,significantly higher than that in advanced cancer 5. 64±2.86 （P 〈 0.01） . In well- differentiated gastric cancer the apoptosis index was 7. 89± 3. 12, while in the poor differentiated gastric cancer it was 5. 23± 2. 21 （P 〈 0.05）. Conclusion： Telomerase activity in gastric cancer was significantly higher compared with normal gastric tissue;Apoptosis index in gastric cancer was significantly lower compared with normal gastric tissue ;The detection of telomerase activity and apoptosis index had significant value in the early diagnosis of gastric cancer ,with significant value in its clinical staging.
