ABR对葛根素治疗椎基底动脉短暂缺血性眩晕的疗效评估

摘　　要：目的:为了评估葛根素治疗椎基底动脉短暂缺血性眩晕(VBTW)的疗效。方法:选用VBTlV患者62例,年龄40-65岁。经纯音测听、声阻抗测试均正常。选用丹麦Madsen公司2260型脑干诱发电位(ABR)系统测试。以短声刺激,刺激声为交替极性,扫描时间10ms,方波脉冲0．1ms交替短声刺激,感觉级75dBSL,最大输出为120dBpeSPL。叠加1000次,增益至1200次。滤波波带通为100-2500 Hz,分别给予11次/s的常规ABR测试和51次/ s的高刺激率ABR测试。左右耳分别检测。应用葛根素治疗15天后,再用同样方法复查ABR。结果:有51例患者参与了随访。以51次/s ABR与11次/s的ABR差值作为分析参数,治疗前后的组群对比,除了波Ⅰ潜伏期两者无差异外(P＞0．05),其余的波潜伏期及波间期,治疗前均明显长于治疗后,具有统计学意义(P＜0．05)。同一患者将自身的治疗相比,除波Ⅰ外,治疗后的波潜伏期及波间期均较治疗前缩短,具有统计学意义(P＜0．05)。结论:高刺激率ABR可以作为临床上葛根素治疗椎基底动脉短暂缺血性眩晕疗效评估的一种选择。Objective：To determine whether Puerarin is effective in treating vertebrobasilar transient ischemic vertigo （VBTIV）. Methods：VBTIV group is composed of 50 VBTIV patients aged 40-65. It is normal in the test of pure tone audiogram and acoustic impedance. We apply 2260 ABR system which is made by Madsen Company in Denmark.Alternative click is given to the stimulus ear. Parameter： paint time： 10ms pulse： 0.1 ms ; hearing level： 75SL ; the largest output： 120dBpeSPL; repeat：1000-1200 ; filter： 100-2500Hz; stimulus frequency： 11/Msec and 51/Msec. In VBTIV group, ,after the patients are treated with Puerarin for fifteen days, their syndromes are improved. We examine ABR with the same method again. Results：There are 51 patients in follow-up studies. We take the distinction of 51/sec ABR and 11/sec ABR regarded as the parameter. Except for wave I peak latency, after the patients are treated for fifteen days, the wave peak latency and wave interpeak latency are shorten than before（P 〈 0.05）. When we make use of self-controlled trial in the patients, results are the same as group-controlled trial. Conclusions： ABR with high stimulus rates can be a choice for Puerarin treatment which is treating vertebrobasilar transient ischemic vertigo.
