腹腔镜和开腹结直肠癌根治手术患者外周血中CK20mRNA的表达及意义

摘　　要：目的研究结直肠癌患者外周血角蛋白20(cytokeratin20,CK20)mRNA的变化,探讨腹腔镜在结直肠癌手术治疗中的应用价值。方法采用反转录-聚合酶链(RT-PCR)法检测37例行根治手术治疗的结直肠癌患者和8例非恶性病变腹部手术患者的外周血CK20mRNA的表达。结果8例良性病变腹部手术患者外周血CK20mRNA无明显表达。37例结直肠癌患者总的CK20mRNA阳性表达率为39.6%(44/111),且术前、术中和术后CK20mRNA的阳性表达率比较差异无统计学意义(P>0.05)。16例开腹手术组和21例腹腔镜组术前、术中、术后相比,各组间CK20mRNA阳性表达率差异亦无统计学意义(P>0.05)。结论开腹手术和腹腔镜手术2种手术方式对CK20mRNA在结直肠癌患者的外周血中的表达差异无统计学意义。Objective To study the alteration of CK20mRNA in the peripheral blood samples by laparoscope and open radical operation of colorectal carcinoma micrometastasis, and investigate the value of laparoscopic surgery in with colorectal cancer . Methods The expression of CK20mRNA in the peripheral blood samples of 37 patients with colorectal carcinoma and 8 patients with non-malignant disease by operations on abdomen. Results The expressions of CK20mRNA are negative in the peripheral blood samples of 8 patients with non-malignant disease by operations on abdomen. Meanwhile, the total positive rate of expression with CK20mRNA are 39.6% （44/ 111 ） in the patients with colorectal carcinoma. The positive expression rates of CK20mRNA in patients of the two groups pre-, during and post-operation do not have significant difference （P 〉 0.05）. Conclusion There are no significant differences in the efficacy of radical operation in patients with colorectal carcinoma by laparoscope and open radical surgery.
