摘 要:目的:探讨鼻咽癌细胞增殖活性指标有丝分裂指数(mitotic index,MI)、增殖细胞核抗原(proliferating cell nuclear anti—gen,PCNA)、p53蛋白、DNA含量和癌细胞核形态参数与放射敏感性的关系。方法:收集51例放疗前鼻咽癌组织标本,HE染色作病理组织学诊断和MI计数;免疫组化法检测PCNA和p53蛋白表达情况;Feulgen染色和图像分析法检测鼻咽癌DNA含量和细胞核的形态定量参数。结果:51例鼻咽癌组织中,完全消退组MI高于残留组(P<0.05);PCNA和p53蛋白表达阳性音分别有50例(98.0%)和46例(90.2%),高表达者分别有16例(31.4%)和22例(43.1%);PCNA和p53蛋白的阳性表达水平越高,则鼻咽癌对放射治疗越敏感,放疗后鼻咽局部癌肿完全消退率越高(P<0.05);PCNA和p53蛋白同时高表达者比单一指标高表达者,对其放射敏感性更高(P<0.05);鼻咽癌完全消退组DI、DNA异倍体细胞所占百分率、细胞核形态参数面积、周长和直径均比残留组高。结论:MI、PCNA、p53蛋白、DNA含量和癌细胞核形态参数是反映鼻咽癌放射敏感性和预测其近期疗效的重要指标。Objective.To investigate the relationship among mitotic index (MI) ,PCNA, p53 protein, DNA content and nuclear morphologic parameters which were the factors of cell proliferative activity of nasopharyngeal carcinoma(NPC) and radiosensitivity. Methods.51 biopsy specimens prior to radiotherapy of NPC were selected. Histopathological diagnosis was made and mitotic index was counted under microscopy by HE staining method. PCNA and p53 protein expressions were detected by immunohistochemical method. DNA content and nuclear morphologic parameters were evaluated by Feulgen staining and image analysis method. Results. In 51 cases of NPC, the MI in the group of no residual tumor was 3.11 ± 2.05, while in the group of residual tumor was 1.95 ± 1.23. The difference between the two groups was significant (P〈0.05). The positive expressions of PCNA and p53 protein were demonstrated in 50 (98.0%) and in 46 (90.2%), respectively. The high positive expressions of PCNA and p53 protein were found in 15 (31.40%) and in 18 (43.1%) out of 51 cases, respectively. The positive expressions between PCNA and p53 protein were highly correlated (P〈0. 01). The higher positive expressions of PCNA and p53 protein, the higher radiosensitivity and rate of complete extinction of NPC locally (P〈0.05). Furthermore, the higher radiosensitivity of NPC was found in the cases with higher positive expressions in both PCNA and p53 protein rather than in those with either PCNA or p53 protein high expression(P〈0.05). The DI, the aneuploid percentage , the nuclear area, perimeter and diameter in the group of no residual tumor were higher than those in the group of residual tumor(P〈0.05). Conclusion: MI, PCNA, p53 protein, DNA content and nuclear morphologic parameters were the important indicators reflecting the radiosensitivity and predicting the radiotherapeutic effect of NPC locally.
