复方甘草酸苷在肝动脉栓塞化疗时保肝作用的临床观察

摘　　要：目的:观察复方甘草酸苷在肝动脉栓塞化疗(TACE)时的保肝作用。方法:69例原发性肝癌患者随机分为对照组与治疗组。对照组于TACE当天开始仅给予支持治疗,治疗组在对照组治疗基础上加用复方甘草酸苷(160mg.d-1)保肝治疗,连续用药14d。结果:治疗组治疗前、后肝功能指标水平无变化,TACE结束后第3天、第7天对照组总胆红素均显著高于治疗组(P<0.01),TACE结束后第14天有下降,但仍高于治疗组(P<0.05);TACE结束后第3天、第7天对照组丙氨酸氨基转移酶和天门冬酸氨基转移酶较治疗组明显升高(P<0.01),TACE结束后第14天降至正常;对照组白蛋白与治疗组比较,TACE结束后第7天开始明显下降(P<0.05),TACE结束后第14天有回升,但未至正常水平(P<0.05)。结论:复方甘草酸苷可防止正常肝细胞损害,保护肝功能,增强患者对栓塞化疗的耐受性。OBJECTIVE： To observe the hepatoprotective effect of Compound glycyrrhizin on patients undergoing transcatheter hepatic arterial chemoembolization（TACE）.METHODS：A total of 69 patients with primary hepatoeellular carcinoma （PHC） were randomized to trial and control group.The control group received supporting treatment alone from the first TACE day, while the trial group received Compound glycyrrhizin iniection 160mg · d^-1 in combination with supporting treatment for 14 consecutive days.RESULTS： The trial group showed no significant differences in parameters of liver function before and after receiving TACE .On days 3 and 7 after the completion of TACE, the total bilirubin level in control group was significantly higher than in trial group（P 〈 0.01）, but slightly reduced on days 14, yet it was still higher than in the trial group （P 〈 0.05） .On days 3 and 7 after the completion of TACE, the ALT and AST levels in the control group（P〈0.01） were significantly higher than in the trial group（P 〈 0.01）, but reduced to the normal level on days 14; the ALB level began to decrease significantly from day 7 after the completion of TACE（P〈0.05）, but rebounded slightly on day 14, yet it failed to return to the normal level（ P 〈 0.05） .CONCLUSION：Compound glycyrrhizin can prevent hepatocellular damage, protect liver function and enhance patients＇ tolerance to TACE.
