摘 要:[目的]探讨X射线工作者染色体畸变与外周血象异常的关系。[方法]采用1:1配对病例-对照研究方法,选择从事X射线工作、工龄>1年并且出现染色体畸变的人员为病例,以与病例在同一单位且在同一岗位工作、工龄相同或比病例长1年以内、同性别、同民族、年龄相差不超过2岁、染色体正常的人员为对照。用微量全血培养法制备染色体标本,镜下观察染色体畸变情况;仪器法检查外周血象;运用Stata软件包进行统计分析。[结果]病例组和对照组WBC分别为(5.77±0.13)×109/L和(6.25±0.14)×109/L,单核细胞(W-SCR)分别为(0.37±0.01)×109/L和(0.34±0.01)×109/L,差异均有显著性(P<0.05);相关性分析表明,染色体畸变率与W-SCR、RBC和Hb呈正相关,W-SCR相关系数有统计学意义(P<0.05);病例组WBC异常率(10.48%)高于对照组(2.86%)(P<0.05)。[结论]辐射导致的染色体损伤与外周血象改变两者之间有关联,发生染色体畸变的放射人员造血系统损伤更严重,白细胞异常率增加。单核细胞数越多染色体畸变率越高。[ Objective ] To explore the relationship of chromosome aberration in peripheral lymphocyte and the abnormal hemogram for X-ray workers. [ Methods ] In a case-control study, 105 workers exposed to X-ray with abnormal chromosome were selected as cases and another 105 X-ray workers with normal chromosome who matched with the cases in sex, age( ± 2years ), work place, and work history (± lyear ) were determined as controls. By means of the culture of a little whole blood, the samples of chromosome were prepared and observed under miroscope. The hemogram was analyzed by F-820 analyzer. The Stata software was used to analysis the data. [ Results ] The average white blood cell count in the case group was lower than that of the control group, while the mean for W-SCR was higher, both had significant difference ( P〈0.05 ). There was correlation between the rate of chromosome aberration and W-SCR( P〈0.05 ). The abnormal rate of WBC in the case group was significant higher than that of the control group( P〈0.05 ). [ Conclusion ] There is a relationship between chromosome aberration and WBC, thai is, the rate of abnormal WBC of workers with chromosome aberration increased after long term exposed to X-ray. The blood system was more severely harmed in X-ray worker with chromosome aberration. The rate of chromosomal aberration was positively related to W-SCR.
