射频热凝固治疗不同程度子宫颈上皮内瘤样病变的价值

摘　　要：目的探讨射频热凝固微创技术(RHMIT)对不同程度子宫颈上皮内瘤样病变(CIN)患者进行保守治疗的疗效,以建立规范化诊疗和随访程序。方法对113例经宫颈刮片细胞学检查和电子阴道镜下活检病理证实的不同程度CIN患者(其中CINⅠ患者52例,CINⅡ47例,CINⅢ14例),进行了RHMIT保留宫颈治疗。本资料严格规范诊疗和随访制度,本组随访时间最短为6个月,最长为47个月,平均为32个月,现仍在随访中。结果本组在应用RHMIT和辅助治疗后,各程度CIN病变的宫颈治疗面在第4～第7周时,已全部愈合。随访期间共进行了586次宫颈细胞学检查提示,未见异型细胞;阴道镜下宫颈活检256次并病理学检查,未见异型细胞。结论RHMIT是一种简单、快速和安全的热疗微创方法,对不同程度CIN有明确疗效。该法依据如下适应证,CIN诊断明确,适合个体化治疗方案,以及规范化随访。Objective To investigate the curative effect and standardization of the treatment method to cervical intraepithelial neoplasia （CIN） by radiofrequeney heat-coagulation minimally invasive treatment （RHMIT）. Methods One hundred and thirteen patients with CIN in different degree（fifty two patients with CIN Ⅰ , forty seven ones with CIN Ⅱ and fourteen ones with CIN Ⅲ ）,who were all confirmed by thinprep cytology test （TCT）, electric eolposeopy and cervical biopsy pathology, were treated by RHMIT method in order to maintain cervical integrity. All patients were strictly followed up from six months to 47 months, 32 months in average. Results All patients with CIN in different degree in this group were fully recovered within 4- 7 weeks after the RHMIT and supplementary treatment. During this following up stage, five hundreds and eighty-six times of TCT and two hundreds fifty six times of cervical biopsy pathology under electric eolposeopy were all not found cellular atypieality. Conclusion RHMIT is a simple, safe, fast, effective and repeatable method to CIN in different degree. The indications of RHMIT to CIN are definite diagnosis, standardizing plan according to individuality and strict followingup programme.
