应用帕金宁控释剂治疗帕金森病伴有运动波动的前瞻性研究

摘　　要：应用帕金宁控释剂（ｓｉｎｅｍｅｔＣＲ）治疗帕金森病伴有运动波动患者４６例，进行了双盲、随机、对照的前瞻性研究。研究结果表明帕金宁控释剂的每日剂量较帕金宁普通片（ｓｉｎｅｍｅｔ）增加２６．０％，每日用药次数减少３３．３％。服用帕金宁控释剂后帕金森病的症状和体征有改善，对运动波动特别是剂末现象和开关现象有效，有效率分别为９４．４％和９０．５％。早晨服用帕金宁控释剂后，由于缓释关系较帕金宁普通片起效慢，故建议早晨加服帕金宁普通片。帕金宁控释剂的副作用与帕金宁普通片相似，同时与剂量大小有关。认为帕金宁控释剂是一种安全有效的抗帕金森病伴有运动波动的药物。Objectivecontroledreleasecarbidopa/levodopa(sinemetCR)andstandardsinemetwerecomparedinaprospectivestudyonthetreatmentof46patientswithParkinson′sdiseaseandmotorfluctuations.Methodsthedouble-blind,randomized,controledandprospectivestudywascarriedinthisclinicaltrial.ResultstheresultsofthisclinicaltherapeutictrialofsinemetCRshowedthatthedailyintakeoflevodoparequiredwasincreasedby26.0%withsinemetCR,whileitsdailydosingfrequencywas33.3%les.AftersinemetCRtreatmenttherewasimprovementinneurologi-calsymptomsandsignsandmotorfluctuationsofParkinson′sdiseaseinbothgroups.Amongthemo-torfluctuationstheon-ofphenomenonandendofdoseweremarkedlyreducedwithsinemetCR.Theireficacyrateswere94.4%and90.3%respectively.ThedelayedresponsetosinemetCRmightbeamelioratedbysupplementingtheinitialdailydosewithstandardsinemet.Theside-efectsofsinemetCRweresimilartostandardsinemetandalsorelatedtodoseoflevodopa.Therewerenore-markablechangesinbloodandurineexamination,bloodbiochemistryandEKGexamination.Conclu-sionitsuggeststhattreatmentofParkinson′sdiseasewithsinemetCRisofvaluebothforitsin-creasedeficacyanditsdecreasedmotorfluctuations.
