胶质瘤中HIF-1α的表达及其与预后关系

摘　　要：目的探讨HIF—1α在脑胶质瘤中表达及其与预后关系。方法采用免疫组化方法检测胶质瘤标本中HIF—1α的表达情况,分析HIF-1α与胶质瘤临床病理特征及预后关系。结果胶质瘤标本HIF-1α表达呈阳性反应,胶质瘤标本HIF-1α阳性表达率为71.67%(43/60),主要位于细胞质和(或)细胞核,具有明显的异质性;肿瘤边缘部份的瘤细胞表达明显增强;高级别胶质瘤(Ⅲ一Ⅳ级)中HIF-1α阳性表达率明显高于低级别者(Ⅰ～Ⅱ级)(x2=18.123,P<0.01);表达强度与病理级别间呈正相关(rs=0.620,P<0.01),HIF-1α阴性、弱阳性、中度阳性和强阳性表达者术后平均生存时间分别为46.11±3.54、37.99±3.72、23.27±2.76和16.96±4.32个月;组间比较有显著差异(x2=22.190,P<0.01)。结论HIF-1α在胶质瘤组织中存在过度表达,胶质瘤HIF-1α的表达强弱与肿瘤病理分级具有关。HIF-1α表达与胶质瘤患者术后生存期呈负相关。Objective In order to investigate the expression of hypoxia inducible factor-α （HIF-α）α and its relationship with prognosis of patients with gliomas. Methods The expression levels of HIF-α in gliomas were detected by immunohistochemistry assay, and analyzed the relationship of the HIF-α expression to clinical pathologic characters and prognosis of the patients. Results There was a positive HIF-α expression in the glioma specimens, and the positive HIF-α expression rate for the glioma specimens was 71.67% （43/60）. h was mostly observed in the cytoplasm and （or） the nucleus with obvious heterogeneity, Enhanced expression could be clearly seen in the surroundings of the edges of the infiltration areas of the tumor, h was shown that HIF-α expression in gliomas with higher pathological classification grade （Ⅲ-Ⅳ） was much more evident than those with lower pathological classification grade （Ⅰ-Ⅱ ） （X^2=18.123, P〈0.01）. The expression intensities were positively correlated with the pathological grades （rs=0.620, P〈0.01）; The mean postoperative survival times of HIF-α （-）, （＋）, （＋＋） and （＋＋＋） groups were 46.11±3.54,37.99±3.72, 23.27±2.76 and 16.96±4.32 months respectively. There was significantly difference in the mean postoperative survival times among different groups （X^2=22.190, P〈 0.01）. Conclusions There were overexpressions of HIF-α in gliomas, and the HIF-α expression intensities were correlated with the glioma pathological grades. The HIF-α expressions were negtively correlated with the postoperative survival times of the patients with gliomas.
