摘　　要：目的讨论上矢状窦旁脑膜瘤的微创治疗方法,以提高手术治疗效果。方法回顾分析我院1996—2005年收治的46例上矢状窦旁脑膜瘤手术治疗经验。结果46例中全切除36例,次全切除10例。4例病人术后—侧偏瘫,—例下肢瘫,随访1—8年,5例复发行第2次手术。结论矢状窦旁脑膜瘤手术效果取决于肿瘤生长部位、侵犯矢状窦的范围。了解矢状窦是否通畅、保护中央静脉及其它引流静脉,显微手术分块切除肿瘤及受侵犯的颅骨、脑膜、静脉窦壁,是根治肿瘤,防止肿瘤复发的重要因素。objective To discuss the method of microsurgical treatment for parasagittal meningiomas in order to improve the curative effection. Methods Retrospective analysis bad been mad of 46 parasagittal meningiomas treated in our hospital from 1996 to 2005. Results 36 cases were totally removed, 10 cases gditotaUy removed. 3 eases developed hemiparalysis and 1 ease paralysis of both lower-extremities 46 patients were followed-up for 1-8 years. 6 recidivation and 5 reoperatian. Conclusions The surgical effect of parasagittal meningioma was made decision the location of the tumor and the extend of the sinus invaved by the tumor know of the sngittal sinus whether blocked or free, protection of the vein of central sulclus and other draining veins, todally or partly to remove the tumor, including the margin of the dura and involved bone and sinus wall using microsurgical technique are the important factors to attain radicalcure and eliminate recurrence.
