摘  要:目的了解P53、GST-π、P170和鼠抗人MDR相关蛋白(MRP)在眼眶腺样囊性癌(ACC)组织中的表达情况。方法免疫组织化学法检测51例眼眶ACC标本和10例正常眼眶组织中P53、GST-π、P170和MRP的表达状况。结果P53、GST-π和P170在眼眶ACC组织中表达阳性率分别为52·94%、100%和82·35%,与眼眶正常组织相比差异有统计学意义。P53和P170在实体型ACC中的表达率明显高于其在腺样-管状型中的表达率;P53在复发组中的表达率明显高于其在初发组中的表达率;各指标之间在表达上无相关性。结论眼眶ACC的多药耐药性与P53、GST-π和P170的过表达有关,但作用机制不尽相同,各指标的检测对于判断预后有参考价值。Objective Adenoid cystic carcinoma(ACC) is a common malignant tumor in the orbit. In the present study,we investigated the expression and clinical significances of the D53, P-glycoprotein (P170) , glutathione S-transferase (GST-π) and multidrug resistance associated protein(MRP) in orbital ACC to provide the theoretical information and clinical reference. Methods Fifty-one samples of ACC determined by pathohistology were collected, and 10 normal orbital tissue samples were obtained during the operation for other benign orbital tumor. The expression of P53, GST-π, P170 and MRP in orbital ACC and normal orbital tissues was detected by immunohistoehemisry. Results D53,GST-π and PI70 were expressed in orbital ACC, displaying brown-yellow granule in cellular nuclear, cellular membrane and cytoplasm respectively with a expression percentage 52. 94% , 100% and 82.35% respectively, showing a higher expression rate in compared with normal orbital tissues( 10% , 10% , 30% , X2 = 7. 128, 44. 546, 10. 455, P = 0. 008, 0. 000, 0. 001 ). The positive rates of P53 and P170 expression in adenoidtubular ACC(30% and 70% )were lower than in solid ACC(85.71% and 100% , x^2 = 16. 648, 10. 880, P =0. 000, 0. 001 ). The positive rate of P53 expression in recurrent tumor group was higher than in initial tumor group(84. 62% versus 42.11% , x^2 =7.634, P =0. 006) ;while that of PITO had not significant difference between recurrent tumor group and initial tumor group (P =0. 242), and GST-π was strongly expressed in all recurrent tumor, initial tumor, adenoid-tubular ACC and solid ACC samples. No MRP was expressed in both ACC tissue and normal orbital tissue. Conclusion The hyperexpression of D53, GST- π and P170 participates in the pathogenesis of acquired MDR in orbital ACC by different pathways. These indexes are of great value in the prognosis evaluation of ACC.
