Survivin基因的表达在胃溃疡及胃癌的研究

摘　　要：目的:研究胃癌(gastric cancer,GC)及胃溃疡(gastric ulcer,GU)组织中生存素(Survivin)的表达规律,探讨胃溃疡在胃癌形成过程中的可能机制,为临床应用以生存素为靶点的胃癌防治措施提供理论依据。方法:采用免疫组织化学染色(SP法)检测不同胃黏膜组织中Survivin的表达。结果:从CSG→CAG→Dys→GC过程中,无论在非溃疡组,还是在溃疡组中,Survivin表达均呈递增趋势。在非溃疡各组中灰度值CSG57.64±4.44、CAG55.05±6.97、Dys48.59±4.16;在溃疡各组中灰度值CSG58.25±5.54、CAG53.95±3.48、Dys48.91±4.63;在胃癌中32.25±4.13。Survivin表达在非溃疡组和溃疡组所对应的胃黏膜比较差异无统计学意义(P>0.05)。而CSG、CAG、Dys、GC之间表达有统计学意义(P<0.05)。胃癌组织中Survivin的表达与年龄、性别、肿瘤大小、临床分期和组织分化程度无关。结论:①Survivin基因表达可作为在胃黏膜由浅表向萎缩、不典型增生和胃癌发展过程中的有用指标。②胃溃疡的癌变实质上是溃疡边缘背景病变萎缩、不典型增生的癌变。Objective：To investigate the expression of Survivin in gastric cancer（GC） and gastric ulcer（GU） and explore the possible roles of GU in the genesis of UC, for provide evidences for the prevention and cure of GC basec on survivin. Methods：The expression of Survivin in various gastric mucosas was detected by immunohistochemistry methods（sp methods）. Results： In CSG, CAG, Dys and Gc groups eith or without ulcer, the expreeion of Survivin was increasing. In groups witbout ulcer, the value of gray scale was 57.64 ± 4.44 in CSG group, 55.05 ± 6.97 in CAG group and 48.59 ± 4.16 in Dys group. In groups with ulcer, the value of gray scale was 58.25± 5.54 in CSG group, 53.95 ± 3.48 in CSG group 48.91 ± 4.63 in Dys group and 32.25 ± 4.13 in GC group. The expression of Survivin showed no difference between groups without ulcer and ones with ulcer（P 〉0.05）, showed differences among CSG, CAG, Dys and GC group（ P 〈0.05）, which was not related to aye, sex, size of tumor, clinical staging and the degrees of differentiation. Coneulion： ①The expression of Survivin gene may be valuable index for predicting the development of gastric mucosal lesions from superficial lesion to atrophy, atypical hyperplasia and GC. ② The cancertion of GU is virtually the carcinoma tous changes in atrophic or atypical hypeplastic tissues at the edge of ulcer.
