硬膜外和腰大肌肌沟两种阻滞治疗坐骨神经痛的临床观察

摘　　要：本文对４４０例神经根性坐骨神经痛病人采用硬膜外阻滞，用药配方为类固醇、维生素Ｂ族和利多卡因。对１２７例神经干性坐骨神经痛病人采用腰大肌肌沟阻滞，用药配方为１．５％利多卡因和０．１％丁卡因混合液。按ＶＡＳ法及直腿抬高试验评分，分级分析疗效。硬膜外阻滞者总优良率为８５．２２％，腰大肌肌沟阻滞者总优良率为７４．８０％。腰大肌肌沟阻滞操作简便，疗效较好，适合于在门诊处理大量病人，是神经干性坐骨神经痛治疗的首选疗法。This paper presents 440 cases of radicular sciatica treated by epidural block with steroid + vitam in B + lidoca in e,and 127 cases of neuroplexus sciatica treated by psoas compartment block with 1.5% lidocaine+0.1% tetracaine. Both effects were scored and analysed according to VAS and straight leg test.The overall satisfactory rate of the epidural block was 85.2%,and that of the psoas compartment block 74.8%.The psoas comport ment block is easy to Perform and shows a fairy good therapeutie effect,therefere can be considered as the first choice for neuroplexus sciatica.
