摘 要:目的探讨脑肿瘤在MR扩散张量成像(DTI)的特点以及DTI在脑肿瘤中的诊断和鉴别诊断价值。资料与方法对60例病理证实的脑肿瘤患者(脑膜瘤17例,胶质瘤24例,神经鞘瘤7例,转移瘤12例)术前行3.0TMRDTI,重建成各向异性(FA)、表观扩散系数(ADC)图和三维纤维束成像,选择肿瘤实质为兴趣区(ROI),测量FA和ADC,进行单因素方差分析。结果脑膜瘤FA最高,其次是神经鞘瘤,再次为胶质瘤,转移瘤最低。FA在脑膜瘤与胶质瘤和转移瘤、神经鞘瘤与胶质瘤和转移瘤、胶质瘤与转移瘤均有统计学意义(P<0.05),脑膜瘤与神经鞘瘤间的FA无统计学意义。神经鞘瘤的ADC值明显高于脑膜瘤、胶质瘤和转移瘤,具有统计学意义(P<0.05)。结论DTI对脑肿瘤具有很高的临床应用价值。Objective To investigate the DTI (diffusion tensor imaging, DTI) findings of intraeranial tumors and to assess its value in diagnosis of intracranial tumors. Materials and Methods 60 patients with tumors proved histopathologically ( 17 meningiomas, 24 gliomas, 7 schwannomas, and 12 metastatic tumors) were examined using DTI with 3.0 T MR scanner before operation. FA images, ADC images and 3D fiber tracking images were reconstructed. ROIs were put in the parenchyma of tumors, the FA and ADC value of the ROIs were recorded. One way anova method was used in statistic analysis. Results Among the four different intracranial tumors, meningiomas had the highest FA, followed by schwannomas, gliomas and metastatic tumors. There were statistic differences in FA between meningiomas and gliomas or metastatic tumors, chwannomas and gliomas or metastatic tumors, gliomas and metastatic tumors ( P 〈 0.05), but no significant difference in FA between meningiomas and schwannomas was shown. The ADC values in schwannomas was statistically higher than that in meningiomas, gliomas, and metastatic tumors ( P 〈 0.05). Conclusion DTI is of high practical value in diagnosis of cerebral tumors.
