前列腺疾患、性生活与前列腺癌危险性关系的病例-对照研究

摘　　要：目的:了解前列腺癌发病的影响因素,为前列腺癌的科学防治提供理论依据。方法:对经病理确诊的前列腺癌40名及对照168名(其中48例医院其他癌对照,60例医院非癌对照和60例正常邻居对照),在湖北省采用问卷方式对婚姻、性生活及既往前列腺疾患(既往病历记录)等进行调查,应用SPSS 13.0软件对资料进行分析。结果:前列腺疾病(前列腺炎及前列腺增生)病例与3种类型对照比较,有明确的统计学关联,前列腺增生OR1=6.4,OR2=3.9,OR3=3.2,P<0.05,前列腺炎OR1=3.7,OR2=6.3,OR3=4.7,P<0.05。首次遗精年龄以13-17岁为基线水平(OR=1.0),≤12岁年龄组3种对照OR分别为1.65,2.18,1.56,有统计学差异(P<0.05);首次性生活年龄以19-29岁为基线水平(OR=1.0),≤15岁组3种类型对照OR分别1.60,1.35,1.89,有统计学差异,P<0.05。手淫习惯,病例组与3组对照比较OR分别为1.70,1.88,2.19,P<0.05。丧偶的病例组与3组对照比较OR分别为1.28,1.14,2.85,P<0.05。结论:有前列腺疾病史、丧偶史,首次遗精年龄、性生活年龄越小,手淫越频繁,则患前列腺癌的危险性越大。Objective： To investigate the risk factors of prostate cancer, for further prevention study of cancer. Methods： A case-control study was conducted for 40 patients with prostate cancer and their matched controls （48 hospitalized cases of other cancers, 60 hospitalized non-cancer cases and 60 normal neighborhood controls） in Hubei province, and all the subjects were interviewed with an identical questionnaire including marriage, sexuality and prostate diseases. The data were analyzed with SPSS 13.0 version. Results： A history of prostatitis or benign prostate hyperplasia （BPH） was associated with a significantly increased risk of prostate cancer （P〈0.05）. Those with the first ejaculation at earlier age （≤12 years）, those with the first intercourse at younger age （≤15 years）, and those with the habit of masturbation were more likely to have prostate cancer as compared to the controls. Loss of spouse might be an increased risk of prostate cancer.Conclusion： The course at earlier history of prostatitis or BPH, loss of spouse, and the first emission and inter age, and the habit of masturbation are risk factors for prostate cancer.
