维持性血透患者上消化道出血的临床分析

摘　　要：目的探讨维持性血透患者并发上消化道出血的原因及临床特点。方法回顾性总结患者的胃镜检查结果及治疗效果。结果30例上消化道出血胃镜检查的结果为胃溃疡6例,十二指肠溃疡6`例,慢性胃炎10例,十二指肠炎5例,广泛性出血性胃炎3例,其中29例治愈,1例死亡。结论胃十二肠指病变是慢性血透病人上消化道出血的原因,因症状多不典型,应常规进行胃镜检查,以便及时出现和妥善处理,有利于防止发生致死性的上消化道大出血。Objective To investigate causes and clinical features of upper gastrointestinal hemorrhage in patients with maintenance hemodialysis. Methods We retrospect gastroscopic results of 30 cases of patients with maintenance hemosialysis accompanied by upper gastrointestinal hemorrhage and analyzed therapeutic effect on them. Results Among the 30 cases of patients, gastroscopy showed that there were 6 cases of gastric ulcer, 6cases of duodenal ulcer, 10cases of chronic gastritis, 5 cases of duodenitis and 3 cases of hemorrhagic gastritis, respectively. 29 cases of them were cured and one died. Conclusion The results showed that gastroduodenal disorders are main causes of upper gastrointestinal hemorrhage in patients with mainehance hemodialysis. For lack of typical gastrointestinal symptoms, patients with maintenance hemosialysis should be given routine gastroscopy, so that gastroduodenal complications in these patients can be found as early as possible and proper treatments are given quickly. These are useful to prevent occurrence of upper gastrointestinal hemorrhage in the patients with maintenance hemodliaysis.
