城镇住院医保患者个人支付费用影响因素的通径分析

摘　　要：[目的]了解医保患者个人支付费用的影响因素,探索降低个人支付费用的策略,为建立有效的费用控制机制提供依据。[方法]以某县内12所医院2004年度所有住院的城镇职工基本医疗保险患者共计1 145例为研究对象,采用通径分析方法,对其个人支付费用影响因素进行分析。[结果]住院日是个人支付费用最主要影响因素;人员类别、医院级别、收入水平直接影响个人支付费用,入院情况通过影响住院日而简接影响个人支付费用;年龄、抢救、手术、癌症、神经系统疾病、循环系统疾病、生育等系统疾病既影响住院日又影响个人支付费用。[结论]需采取综合控制措施来有效控制个人支付费用的增长。[ Objective] To understand the influenceial factors on the individual account of medical insurant inpatients, explore the tactics of individual account control, and offer the evidence for setting up an effective mechanism for medical expenses control. [Methods] The data derived from the information system of the medical insurance during 2004 in 12 hospitals, 1 145 medical insurant inpatients were enrolled in this study, their individual account were analysed with path analysis method. [ Results] It was the hospitalized days that was the critical influential factor of individual account. Personnel sorts, hospital degree, income levels influenced individual account directly, the complexion of entering hospital influenced individual account by way of the hospitalized days. Age, salvage, operation, cancer, disease of nerve system, circulatory system and procreating influeneed the hospitalized days as well as individual account. [Conclusion] The comprehensive measures must be taken to effectively control the individual account.
