胃癌组织P27、MMP-9和PEDF蛋白表达及意义

摘　　要：目的:探讨P27、MMP-9和PEDF蛋白表达在胃癌发生发展中的作用。方法:应用免疫组织化学法检测82例人体胃癌组织标本中胃癌细胞内P27、MMP-9和PEDF的表达;统计学分析上述各分子标记物与胃癌临床病理因素间的关系。结果:(1)82例正常胃黏膜均有P27、PEDF表达,在胃癌组织中的表达率分别为48%和43%。MMP-9在胃癌中表达率为70%,正常胃黏膜呈阴性表达。(2)P27低表达与胃癌组织学分级和远处转移有关(P<0.05);MMP-9高表达与浸润深度、淋巴结转移和远处转移有关(P<0.05);PEDF低表达与组织学分级、浸润深度、淋巴结转移和远处转移有关(P<0.05)。结论:胃黏膜上皮细胞在癌变过程中P27和PEDF表达降低,而MMP-9表达升高;MMP-9在胃癌侵袭转移中起促进作用,P27和PEDF可抑制胃癌的进展。Objective：To investigate the significance of P27, MMP-9 （Matrix Metalloproteinase 9） and PEDF （Pigment epithelium-derived factor） expression in human gastric carcinoma. Methods：Immunohistochemistry method was used to detect the expression of P27, MMP-9 and PEDF in 82 cases of gastric carcinoma tissue samples. The relationship among the molecular markers and clinicopathologic factors was analyzed： Results：（1）In the study, P27 and PEDF existed in almost normal gastric mucous, the expression rates of P27 ,PEDF and MMP-9 were 48 %,43% and 70 respectively in the gastric carcinoma tissues. （2） P27 expression was related to histological grade of tumors and distant metastasis. MMP-9 expression was related to depth of invasion and lymph node metastasis as well as distant metastasis. Low PEDF protein expression was significantly associated with poor tumor differentiation, depth of invasion, lymph node and distant metastasis. Conclusion： The gastric mucosa epithelial cells will decrease the expressions of P27 and PEDF and increase the expression of MMP-9 when transforming to malignant cells. MMP- 9 can accelerate the development and progression of gastric carcinoma, while P27 and PEDF play a negative role in tumorgenesis and progress.
