慢性乙型肝炎肝纤维化程度与脾静脉直径、脾脏大小相关关系探讨

摘　　要：目的 探讨慢性乙型肝炎肝纤维化程度与脾静脉直径（SPV）、脾脏厚度（SPT）和脾脏长径（SPL）的关系。方法 采用回顾性分析的方法，对155例已行肝组织病理检查并按肝组织纤维化分期分为S1～S4期的慢性乙型肝炎患者，分别对其B超测量的脾厚、脾长径、脾静脉直径进行统计分析；并比较各组患者上述几种指标的差异，同时将肝纤维化程度分别与脾厚、脾长径、脾静脉直径之间进行相关关系分析。结果 随着肝纤维化程度的加深，脾脏的厚度、脾长径、脾静脉直径逐渐增大；各纤维化分期的脾厚、脾长径、脾静脉直径之间差异均具有显著性（P〈0．05）。经相关关系分析，这三方面的变化分别与肝纤维化分期具有正相关关系（P〈0．01）。结论 随着肝纤维化程度逐渐加重，慢性乙型肝炎患者脾静脉直径逐渐扩张，脾脏逐渐增大。脾脏大小、脾静脉直径与肝纤维化之间具有正相关关系。Objective To study the relationship between the liver fibrosis stages and the diameters of spleen vein （SPV）, spleen length （SPL）and spleen thickness （SPT）in patients with chronic hepatitis B. Methods A retrospective analysis of 155 cases with chronic hepatitis B were taken. All cases have experienced the liver biopsy. The diameters of SPV and the SPL as well as SPT were compared in different fibrosis stagings of these cases, respectively. Meanwhile the correlationship was analyzed between the liver fibrosis stages and the diameter of SPV, SPL as well as SPT. Result The value of SPT, SPL as well as the diameter of SPV elevated graduately as the increasing of fibrosis stages. The group of $4 was significahtly different among the group of S1, S2 or S3 （P 〈 0.05 ）respectively. In addition, the fibrosis stages had positive correlation with the diameters of SPV, SPL as well as SPT, respectively （P 〈 0.01 ）. Conclusions With the progression of liver fibrosis, the diameters of SPV and the SPT as well as SPL increase graduately in patients with chronic hepatitis B. The liver fibrosis stages has positive correlation wlth the SPV diameters and the SPT as well as SPL, respectively.
