长春瑞滨联合顺铂、氟尿嘧啶/亚叶酸钙治疗晚期食管癌的临床观察

摘　　要：目的观察以长春瑞滨为主的联合化疗方案治疗晚期食管癌的疗效和安全性。方法经病理学确诊的晚期食管癌病人64例,随机分为两组,每组32例。治疗组采用长春瑞滨25mg/m^2,静滴,第1、8d;顺铂30mg/m^2,静滴,第2～4d;亚叶酸钙0.2及氟尿嘧啶0.5g/m^2,静滴,第1～5d。对照组除无长春瑞滨外,皆相同。每3周重复1次,连用3周期评定疗效。结果治疗组和对照组的有效率分别为62.5%和43.8%,在统计学上有显著差异(P<0.05);中位生存期分别为11.3个月和8.7个月;主要毒副作用为骨髓抑制及消化道反应。结论以长春瑞滨为主的联合化疗方案治疗晚期食管,疗效好,毒性小,值得进一步推广。Objective To evaluate the efficacy and safety of combination chemotherapy based on vinorebine in the treatment of patients with advanced esophgeal cancer.Methods Sixty-four cases with advaced esophageal cancer vierified by pathology wer divided randomly into 2groups.Each group consisted of 32 cases,The patients in treatment group were treated winth vinorelbine 25mg/m^2 on days 1 - 8;cisplatin 30mg/m^2 iv on days2- 4;fluorouracil 0.5g/m^2 and CF 0,2 iv on days1- 5,In the control group the administration and dosage of fluorouracil and cisplatin were same as the treatment group,Drugs were repeated every 3 weeks and curative effects were evaluated after 3 cycles.Results The response rates of treatment group and control group were 62,5% and 43.8%,respectively,with significant statistical difference （P〈0.05）.The median survival time of patients in 2 groups was 11,3 months and 8,7 months,respectively.Conclusion The main side effects were neutropenia and digestive tract reaction,It deserves to be further popularized clinically.
