摘  要:本文应用图像分析技术,研究38例喉鳞癌和29例喉部良性病变组织中,细胞DNA倍体分布状况,分析了其临床意义。结果显示:①喉癌组2C细胞比良性病变组明显减少,而3C—4C细胞、5C及5C以上(以下简称DNA5C—)细胞比后者明显增加,表明喉癌组织中细胞有较强的增殖活性;②以DNA3C及3C以上(以下简称DNA3C—)细胞百分率为指标,在喉癌的诊断和鉴别诊断中有较好的诊断性能,以DNA3C—细胞百分率大于49%为诊断标准,敏感度达94.74%,特异度达89.66%,使用图像分析仪,以DNA倍体分布为依据,进行喉癌的病理诊断和鉴别诊断,具有一定实用价值;③DNA5C—细胞百分率大于33%的喉癌患者5年生存率比小于33%的患者低。DNA倍体分析对喉癌有一定预后价值。Abstract DNA ploidy of 38 cases of aryngeal squamous cell carcinoma(LSCC)and 29 cases oflaryngeal benign lesions(LBL ) was determined by lmage Process System. The resultshowed:1.The percentage of diploid cells in LSCCs was much lower than that in LBLs,while the percentage of DNA3C-4C cells(of which DNA ploidy were 3 to 4),and DNA5C cells (of which DNA ploidy were 5 or greater than 5 ) in LSCCs was much higher thanthat in LBLs,A stronger proliferating activity of LSCCs than that of LBLs was suggested.2.For diagnostic and differential diagnostic practice, the percentage of DNA3C cells(ofwhich DNA ploidy were 3 or greater than 3) had a superior characteristic, When the per-centage of DNA3C greater than 49%was used for the diagnosis of LSCC, the sensitivitywas 94. 74%, specificity was 89. 66%。 Determination of DNA ploidy by Image Process Sys-tem might be used in the diagnosis and differential diagnosis of LSCC,3; LSCC patientswhose percentage of DNA5C cells was greater than 33%had a lower five year survivalrate than those patients whose percentage of DNA5C cells was smaller than 33%,Thepercentage of DNA 5C cells had prognostic value in LSCCs.
