L-OHP联合5-Fu/LV二线治疗晚期胃癌2周方案与FAM方案远期临床效果比较

摘　　要：目的用氟尿嘧啶持续静滴联合奥沙利铂及亚叶酸钙治疗中晚期消化系统胃癌,与经典FAM化疗方案对比,观察其毒副作用、疗效、缓解期及生存率。方法将60例胃癌患者随机分为2组:A组30例,用氟尿嘧啶持续静滴44 h联合奥沙利铂及亚叶酸钙,2周重复,2次为1个疗程,最少进行4个疗程。B组30例,用氟尿嘧啶+丝裂霉素+阿霉素方案化疗,最少进行4个疗程。结果A组CR 0例,PR 17例,SD 6例,PD 7例,总有效率为57%,中位无病生存期(10.67±1.0)个月,1 a生存率为30%。其毒副作用主要为外周神经毒性,但均可以耐受。而B组CR 0例,PR 10例,SD 8例,PD 12例,总有效率为33%,中位无病生存期为(8.7±0.7)个月,1 a生存率17%;毒副作用主要为不同程度的骨髓抑制、消化道毒性反应,对症治疗后可缓解。结论用氟尿嘧啶持续静滴联合奥沙利铂及亚叶酸钙治疗晚期胃癌,无论有效率、缓解期、中位无病生存期、1 a生存率均明显优于传统FAM经典化疗方案,而毒副作用并没有增加,是一个安全有效且耐受性较好的中晚期胃癌的治疗方案,值得进一步扩大临床研究。Objective It is to compare and observe the side effects, clinical effects, catabasis and survival rate between flurouracil intravenous drip continuously combined with Oxaliplatin and Calcium Folinate and FAM regimen on advanced stage gastric cancer. Methods 60 patients with gastric cancer were randomly divided into two groups. 30 cases in group A were treated with flurouracil intravenous drip continuously for 44 h combined with Oxaliplatin and Calcium Folinate. The regimen was repeated every two weeks and four weeks was a cycle, altogether for four cycles. 30 cases in group B were treated with flurouracil ＋ mitomycin ＋ adriamycin regimen and altogether for four cycles. Results In group A, CR was 0 case, PR was 17 cases, SD was 6 cases and PD was 7 cases, the total effective rate was 57 96, median anosia life span was 10.67 ＋ 1.0 months, the 1 a survival rate was 30 % and the side effect mainly was peripheric neurotoxicity but could be tolerance. In group B, CR was 0 case, PR was 10 cases, SD was 8 cases and PD was 12 cases, the total effective rate was 33%, median anosia life span was 8.7 ± 0.7 months, the 1 a survival rate was 17 % and the side effects mainly were different degree bone marrow depression and digestive tract toxic reaction which could be relieved after symptomatic treatment. Conclusion The total effective rate, catabasis, median anosia life span and 1 a survival rate of flurouracil intravenous drip continuously combined with Oxaliplatin and Calcium Folinate on advanced stage gastric cancer are obviously better than FAM regimen and the side effect was little. It is safe, effective, good tolerance and worth further expanding the clinical study.
