格林-巴利综合征病情与血清TNF-α和IL-6的相关性

摘　　要：目的观察肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)水平在不同程度格林-巴利综合征(GBS)患者早期血清中的变化,探讨其对GBS预后的影响。方法采用ELISA法、放射免疫法测定75例轻型、59例中型、31例重型GBS患者入院第2天血清中IL-6、TNF-α水平,另设30名健康人作对照。同时对出院者再次进行肌力测量评定,观察IL-6、TNF-α不同水平对GBS预后的影响。结果与对照组比较,轻型组血清中IL-6、TNF-α含量明显升高(P<0.01),中型组其含量水平高于轻型组(P<0.01),但低于重型组(P<0.01)。同等条件下GBS患者早期血清中IL-6、TNF-α水平越高,致残率越高(P<0.05)。结论GBS脑梗死患者早期血清中IL-6、TNF-α水平明显升高,病情越严重其含量越高;IL-6、TNF-α含量越高,GBS致残率越高。Objective To observe changes of serum IL-6,TNF-α level in various severity degree of early patients with Guillain -Barre syndrome （GBS）, and to discuss their effects on prognosis of patients with GBS.Methods All the cases were divided into three groups： mild （n=75）,middle （n=59）and severe groups （n=31）;30 cases healthy volunteers were supposed as control group. The serum IL-6 and TNF-α levels were measured on the 2nd day after admission with ELISA and radioimmunoassay. In addition, orgometry was done and used to judge the influence on survival;relation of the serum IL-6,TNF-α level with quality of life of survival prognosis was analysed.Results The serum IL-6,TNF-α level of mild group higher than that of control group （P〈0.01）. The serum IL-6,TNF-α level of middle group were higher or lower than that of mild group or severity group （P〈0.01）. The higher serum IL-6,TNF-α level were, the higher ratio of handicap were in the same condition （P〈0.05）.Conclusion The serum IL-6, TNF-α level of early patients with GBS are higher than that of health volunteers; the severer the condition of patients are, the higher the levels are;the higher the serum IL-6,TNF-α levels are, the higher the ratio of handicap are.
