5-氟尿嘧啶 顺铂方案与FOLFOX4方案治疗晚期胃癌比较

摘　　要：目的评价5-氟尿嘧啶(5-FU)持续滴注联合低浓度顺铂(DDP)及FOLFOX4方案治疗晚期胃癌的疗效及不良反应。方法66例晚期胃癌分为FP组和FOLFOX4组,FP组行DDP 4mg/(m2.d),每周5d,连用3周,5-FU 300mg/(m2.d),持续24h,连用3周。FOLFOX4组行草酸铂(L-OHP)85mg/m2,第1d,甲酰四氢叶酸(CF)200mg/m2,第1、2d,5-FU 400mg/m2静脉推注,第1、2d,5-FU1200mg/m2持续输注44h,每2周为1周期。结果FP组总有效率35.48%,主要不良反应为消化道毒性,FOLFOX4组总有效率40.00%,主要不良反应为神经毒性。结论FP方案与FOLFOX方案均为有效低毒的方案,适用于晚期胃癌患者。Objective To evaluate the efficacy and toxicity of combination therapy of continuous venous infusion of 5 - fluorouracil （5 - FU） and low - dose cisplatin （DDP） in advanced gastric cancer. Methods 66 patients with advanced gastric cancer were divided into two groups：FP group treated with 5 -FU 300mg/（ m^2 · d） c iv, and DDP 4mg/（m^2 · d）, ivgtt qd ,Sd/w, which were used for 3 weeks. FOLFOX4 group treated with oxaliplatin （LOHP） 85mg/m^2, d1, leucovorin （CF） 200 mg/m^2 dl, d2, 5 -FU 400mg,/m^2 iv d1 ,d2, 5 -FU 1200mg/m^2 civ dl ,d2. the treatment schedule was recycled every 14days. Results The total response rate in FP group was 35.48%, The total response rate in FOLFOX4 group was 40. 00%. Major toxic effects of FP group was gastrointestinal tract reaction. Major toxic effects of FOLFOX4 group was neuro - sensory toxicity. Conclusion FP regimen and FOLFOX4 regimen are good effective and have less toxic effects, suitable for advanced gastric cancer.
