双阻板矫治器联合肌激动器矫治下颌后缩畸形

摘　　要：目的探讨用双阻板矫治器（twin block appliance，TBA）联合肌激动器矫治下颌后缩畸形的临床价值。方法24例下颌后缩的骨性Ⅱ类Ⅰ分类错[牙合]患者，分为两组，每组12例，分别采用TBA联合肌激动器和传统的TBA矫治方法进行矫治。患者均在治疗的开始、第1期结束时和第2期结束时摄取X线头颅侧位定位片，用McNamara分析法进行各参数的测量分析。结果两组在矫治的第1期皆能有效改善面型，下颌后缩得到纠正，前牙建立正常咬合关系。在矫治的第2期，TBA联合肌激动器矫治组后牙建殆良好，并能维持第1期达到的前牙正常咬合关系；传统的TBA矫治组在后牙建[牙合]过程中不能很好地维持前牙正常咬合关系，下前牙向后移动，下颌向后向上回复。结论TBA联合肌激动器矫治下颌后缩畸形，较传统的TBA矫治器矫治方法有明显的优势，值得提倡。Objective To evaluate the availability of twin block appliance combined with activator in correction of mandibular retrognatbism. Method A total of 24 cases of developing class Ⅱ division Ⅰ malocclusion with mandibular retrognathism were selected. They were equally divided into two groups. One group was treated using twin block appliance combined with activator （combination group）, and the other using conventional twin block appliance （twin block group）. Lateral cephalograms were analyzed at the start of treatment, right after creating normal anterior occlusion and the end of the overall treatment protocol by means of McNamara analysis. Results At the end of the first phase, facial front view and profile were significantly ameliorative in two groups. Mandibular retrognathism was corrected and anterior teeth occlusion was created in this groups. During the second phase, in combination group, normal posterior occlusion was created and the perfect anterior relationship that obtained in first phase was maintained or improved. The position of the mandible and the anterior teeth moved forward and/or downward in this group. During the second phase in twin block group, when posterior occlusion was created, anterior relationship that obtained in first phase could not be maintained, anterior teeth moved backward, mandibles relapse upward and backward partially. Conclusions The treatment protocol of using twin block appliance combined with activator is recommended for its better treatment effect in correction of mandibular retrognathism, as compared with conventional twin block technique.
