黄体期手术改善激素依赖性乳腺癌患者预后

摘　　要：目的观察雌激素受体(ER)和孕激素受体(PR)表达不同的绝经前乳腺癌患者预后与手术时间的关系。方法376例女性乳腺癌病人,根据ER和PR表达情况,将病人分为激素依赖组和非激素依赖组。激素依赖组包括ER(+)或者PR(+),共304例;非激素依赖组为ER和PR都为(-),共72例。根据手术时间及月经史,激素依赖组和非激素依赖组患者分为黄体期组和卵泡期组。观察所有患者5年无病生存率与5年生存率情况。结果376例乳腺癌患者总5年生存率为76.1%,5年无病生存率为66.0%。激素依赖组患者黄体期手术,5年生存率为88.6%,高于卵泡期手术患者的69.9%(P<0.05);5年无病生存率为75.9%,与卵泡期手术患者的61.6%差异无统计学意义(P>0.05)。非激素依赖组患者黄体期手术,5年生存率为60.0%,5年无病生存率为55.0%,与卵泡期手术患者的62.5%与50.0%相比,差异无统计学意义(P>0.05)。结论黄体期手术有利于激素依赖型乳腺癌患者预后。[Objective] To reveal the relationship between prognosis and operation time in different breast cancer patients which were positive or negative for ER （estrogen receptor） and PR （progesterone receptor）. [Methods] 376 patients with breast cancer were divided into hormonal dependent group （304 patients） and hormonal independent group （72 patients） by the positive or negative of ER or PR. Every group patients were divided into operation in luteal phase group and operation in follicular phase group. The disease free survival rate and survival rate were observed for five years in all patients with breast cancer. [Results] The five-year disease free survival rate was 76.1% in all 376 patients, the five-year survival rate was 88.6%. In hormonal dependent group patients, the five-year disease free survival rate of patients who received operation in luteal phase was 88.6%, which was higher than that of patients which received operation in follicular phase （69.9%）. But the five-year survival rate of patients who received operation in luteal phase was 75.9%, which was insignificant from that of patients which received operation in follicular phase （61.6%）. In hormonal independent group patients, the five-year disease free survival rate and the five- year survival rate of patients which received operation in luteal phase, which were 60.0% and 55.5%, were all insignificant from that of patients who received operation in follicular phase, which were 62.5% and 50.0%. [Conclusion] Operation in luteal phase benefits to breast cancer patients who were positive for ER or PR.
