周围型肺错构瘤的影像表现

摘　　要：目的探讨周围型肺错构瘤的影像表现,以提高诊断正确率。方法回顾性分析经手术病理证实的22例周围型肺错构瘤的影像表现。胸片检查12例,CT扫描16例(其中6例同时有胸片)。结果所有病例均为单发,右肺13例,左肺9例。全部错构瘤病灶均为边缘清楚、无毛刺或卫星病灶的肺内肿块,浅分叶5例,平片显示钙化3例,CT扫描脂肪成分和钙化的显示分别为8例和6例(其中2例呈典型的爆米花样钙化)。7例行CT增强扫描均无明显强化。结论爆米花样钙化及脂肪密度是周围型肺错构瘤的特征性影像表现,薄层CT扫描尤其是HRCT检查能提高肺错构瘤的诊断准确率,增强CT扫描对错构瘤的鉴别诊断有一定的价值。Objective To investigate the imaging findings of peripheral - type pulmonary hamartoma so as to elevate the diagnostic accuracy. Methods The imaging findings of 22 patients with surgically and pathologically proved peripheral - type pulmonary hamartoma （PPH） were retrospectively analyzed. 18 of 22 patients underwent both examinations of chest radiography and CT scan before and after the administration of contrast medium. Of whom, 6 cases underwent only chest radiography and 10 cases underwent only chest CT scan. Results The lesions of PPHs in all patients were single, those on left lung and right lung were found in 13 and 9 cases, respectively. All lesions in the lung parenchyma presented as well- margin masses without spiculation or satellite foci hut those in 5 cases presented the shoal lobulation. Chest radiography showed the calcified foci in 3 cases, while CT scan showed the fatty component and calcification of the lesions in 8 and 6 cases, respectively. Typical pop - corn ball calcification was found in 2 cases. The lesions of PPH in 7 patients who underwent CT enhanced scans no obvious enhanced features were found after venous injection of contrast medium. Conclusion Pop - corn ball calcification and fatty density of the lesions are characteristic imaging findings for PPH. Thin slice CT scan, especially HRCT examinations, can elevate significantly the diagnostic accuracy of PPH, while enhanced CT scan has important value for the differential diagnosis of PPH.
