摘 要:目的探讨线栓法制作兔大脑中动脉局灶性脑缺血模型的最适线栓头端直径,并采用正电子发射断层显像技术(PET)检查缺血脑组织葡萄糖代谢水平的变化。方法将兔龄5~6个月的健康雄性新西兰白兔27只随机分为假手术组和手术组,其中假手术组3只,手术组24只。根据线栓头端直径不同再将手术组随机分为0.25~0.29(A组)、0.30~0.34(B组)、0.35~0.39(C组)及0.40~0.45mm组(D组),每组6只兔。所有兔在手术后30min、3及6h行脑PET检查,6h行神经功能评分。检查结束后,解剖观察并取脑组织做氯化三苯基四氮唑(TFC)染色。结果①有1/3枕动脉起自颈内动脉。②手术组24只兔中,有15只模型成功,线栓进人大脑中动脉,A、B、C及D组插线成功的比例分别为3/6、5/6、5/6及2/6,B、C组与D组比较差异有统计学意义(P〈0.05)。③缺血30min,15只成功模型的PET结果即显示双侧基底核、颞叶、顶叶皮质葡萄糖代谢率的比值(右侧/左侧)下降,且随缺血时间延长下降越明显;似手术组无下降。与假手术组比较,A、B、C、D组比值下降的差异均有统计学意义(P〈0.05,P〈0.01,P〈0.01,P〈0.05)。④A组6h神经功能评分〈2级,TFC染色未发现明显异常:B、C及D组6h神经功能评分≥2级,TTC染色可见明显的梗死病灶。结论制作兔大脑中动脉局灶性缺血模型最佳线栓头径为0.30~0.40mm。PET检查对早期血流量减少很敏感,可作为研究脑缺血的有效评估手段。Objective To explore the most appropriate thread tip in making focal cerebral ischemic model of middle cerebral artery with intraluminal thread approach in rabbits and to evaluate the changes of glucose metabolic level with brain positron emission tomography (PET) imaging technique. Methods Twentyseven healthy New Zealand white rabbits aged 5 to 6 months were randomly assigned to sham-operation group ( n = 3 ) and operation group ( n = 24). According to the different thread tip diameters the operation group was randomly allocated into 0. 25 - 0. 29 ( A group), 0. 30 - 0. 34 ( B group), 0. 35 - 0. 39 ( C group), and 0. 40 -0. 45 mm( D group) subgroups (n =6 in each subgroup). PET examination and neurological function score were performed in all rabbits at 30 minutes, 3 h, and 6 h after operation. The rabbits were anatomized and observed, and their brains were taken out for 3TC staining after the examination. Results (1) About 1/3 occipital arteries originated from internal carotid arteries. (2) Of the 24 rabbits in the operation group 15 threads were inserted into middle cerebral arteries, and the successful rate was 3/6,5/6, 5/6, and 2/6 respectively in the A, B, C, and D groups. (3) After 30 minutes of ischemia, the PET results of the 15 successful models showed that the ratio of glucose metabolic rate decreased in bilateral basal ganglias, temporal and occipital cortexes, and decreased significantly with the ischemic time prolonged. However, they did not decrease in the sham - operation group. There were significant differences in ratio decrease in the A, B, C, and D groups as compared with the control group (P 〈0.05, P 〈0.01, P 〈0.01, and P 〈0. 05). (4) The 6-hour neurological function score was 〈 2 in the A group, and TI'C staining was normal. The 6-hour neurological function score was ≥2 in the B, C, and D groups, and clear infarction foci could be found with TTC staining.
