摘  要:目的:通过应用两种不同方法对离体猪肝及正常人体肝脏行容积测量,对比探讨其准确性与实用性,旨在探索一种较理想的肝容积测量方法。方法:对7例离体猪肝(叶)用溢水法测量其容积,再行CT平扫,比较逐层叠加法、选择性VR自动法测量结果并与溢水法测量结果进行统计比较。收集体检行腹部CT增强检查正常者35例,男22例,女13例,运用逐层叠加法和选择性VR自动测量法测量其肝脏容积,并进行比较。结果:7例离体猪肝(叶)测量结果显示两种方法测量结果与溢水法之间具有良好的相关性(r=0.99)。35例活体人肝两种方法测量结果分别为(1371.85±238.01)cm3(、1380.16±234.16)cm3,测量结果间无统计学差异(t=-1.099,P=0.281>0.05);平均耗费时间分别为(27.93±5.55)s(、6.66±1.88)s,选择性VR自动法明显短于逐层叠加法(t=18.31,P<0.001)。结论:选择性VR自动测量法与常规逐层叠加法相比,测量结果准确可靠,且显著减少测量时间,故具有更大的临床实用价值,值得进一步推广使用。Objective: To explore an ideal liver volume measurement methed by using two different methods in measuring the volume of vitro procine liver and human liver.Methods:(1) The volume of seven vitro porcine liver or lobe were measured by means of water-displacement, then CT scan were performed and hand tracing measurement、VR automatic method were used to calculate the liver (or liver lobe) volume. The results were compared by SPSS. (2)The liver volume of thirty five cases underwent healthy examination with enhanced MDCT were measured by hand tracing method and VR automatic method , the volume results and consuming time of the two methods were compared. Two-tailed paired t-test, correlation coefficient were used for statistical analyses.Results: The volume estimated by two methods had a good correlation with the result measured by water-displacement in seven vitro porcine liver or lobe. The mean volume of human liver measured by the two methods were (1371.85 ± 238.01)cm^3,(1380.16±234.16)cm^3, there was no significant difference between the two group results using paired t-test(t = - 1.099, P = 0.281 〉 0.05) ; the consuming time were(27.93 ± 5.55)s ,(6.66 ± 1.88) s respectively,there was significant difference between them( t = 18.31, P 〈 0.001 ). Conclusion: VR automatic method can provide acceptable rest, Its and significantly reduce the time required for volumetry of the liver. It was an ideal measure method for liver volume and had a great clinical value.
