摘  要:目的探讨胸苷磷酸化酶(TP)在肿瘤细胞中的表达及与卡培他滨临床疗效之间的关系。方法33例恶性肿瘤患者(乳腺癌12例,结直肠癌21例)均用过卡培他滨治疗并有临床评价,用免疫组化方法回顾性检测患者治疗前肿瘤组织石蜡标本中胸苷磷酸化酶的表达,比较卡培他滨临床疗效与肿瘤细胞中TP表达阳性率之间的关系。结果乳腺癌TP阳性率83.3%(10/12);结、直肠癌TP阳性率57.2%(12/21)。卡培他滨在乳腺癌和结、直肠癌患者中总的疾病控制率(CR+PR+NC)为63.6%(21/33),其中在TP阳性患者中的疾病控制率为81.8%(18/22),在TP阴性患者中的疾病控制率为27.3%(3/11),二者差异极显著(P<0.01)。结论在乳腺癌和结直肠癌患者中,TP阳性表达的患者对卡培他滨反应更好,疗效更高。为TP表达作为卡培他滨疗效的预测指标提供理论依据。Objective To evaluate the relationship between thymidine phosphorylase (TP) expression in tumor cells and clinical effects of capecitabine. Methods 33 cases of malignant tumors, including 12 breast cancer and 21 colorectal cancer, were all treated by capecitabine, and clinical evaluations were conducted. Thymidine phosphorylase expression was detected in the tumor cells of these 33 patients before treatment with an immunohistochemical method. The relationship was compared between the effects and the expression. Results TP positive rates in breast cancer patients were 83.3% (10/12), and 57.2% (12/21) in colorectal cancer patients. The total tumor control rates (TCRs) for all patients with breast cancer and colorectal cancer treated by capecitabine were 63.6% (21/33). In the TP positive patients TCRs were 81.8% (18/22), whereas in TP negative patients TCRs were 27.3 % (3/11), and the difference had statistical significance (P 〈 0.01 ). Condusion For breast and colorectal cancers, patients with TP positive expression in tumor cells seem more sensitive to the treatment of capecitabine. This pilot study provides evidence to prognosticate capecitabine effects by detecting TP expression.
