主要酸相关疾病演变和幽门螺杆菌感染10年情况分析

摘　　要：目的探讨10年来幽门螺杆菌（Hp）感染及主要酸相关疾病在胃镜检查患者中的变化。方法分析1996年1月至2005年12月进行胃镜检查的所有病例，共计53598例。选择全部十二指肠球溃疡（DU）、胃溃疡（GU）、反流性食管炎（RE）的病例为研究对象。观察却感染情况及DU、GU、RE检出情况的变化。结果10年来却的平均检出率为42．17％。1996年最高，为48．77％，2005年最低，为33．61％，却的检出率呈下降趋势。DU、GU、RE的平均检出率分别为10．87％、3．53％和6．35％；1996年3种疾病的检出率分别15．91％，3．62％和3．53％；2005年3种疾病的检出率分别为8．30％，2．71％和6．06％。DU、GU的检出率均呈下降趋势；RE的检出率呈上升趋势。DU、GU、RE的平均年龄分别为43．72、53．49和55．83岁；1996年3种疾病的平均年龄分别为43．42、50．61和51．92岁，2005年3种疾病的检出平均年龄分别为43．51、55．53和56．89岁。GU和RE的平均检出年龄呈上升趋势，DU的平均检出年龄无明显变化。结论坳的检出率近年呈下降趋势，DU、GU的检出率呈下降趋势；RE的检出率呈上升趋势。GU及RE的检出年龄呈上升趋势，DU的检出年龄无明显变化。Objective To study the changes of Hp infection status and duodenal ulcer（ DU）, gastric ulcer（ GU）, reflux esophagitis （RE） according to the clinical data of endoscopy in our hospital during the past 10 years. Methods The patients with DU, GU and RE＇diagnosed by endoscopy had been selected among the 53 598 cases with endoscopy during the period from Jan. 1996 to Nov. 2005. The changes of Hp infection status and the changes of the detection rate of the 4 diseases were studied. Results The average detection rate of Hp is 42.17%. In 1996, the detected rate of Hp was 48. 77% , which was the highest detection rate. The lowest detection rate was in 2005, which was 33.61%. The detection rate of Hp has decreased during the past 10 years. The average detection rate of DU, GU and RE were 10. 87%, 3.53% and 6.35% respectively. The detection rate of DU and GU has decreased during the past 10 years. The detection rate of RE has increased. The average ages of DU, GU, GC and RE were 43.72, 53.49 and 55.83 years old, respectively. The average age of GU and RE gradually increased. The average age of DU doesnt show ascendent tendency or descendent tendency. Conclusion The Hp detection rate shows descendent tendency. The detection rates of DU, GU decreased. The detection rate of RE increased. The average age of GC and RE increased. The average age of DU was not changed significantly.
