摘 要:目的:介绍神经干细胞移植治疗脑卒中和颅脑损伤的实验和临床效果,并探讨神经干细胞移植应用于临床存在的问题。资料来源:应用计算机检索PUBMED2000/2007与神经干细胞移植治疗脑卒中和颅脑损伤相关的文章,检索词“neural stem cell,stroke,brain damage”,限定文献语种为“English”;同时检索万方数据库2000/2007相关文章,检索词“神经干细胞,脑卒中,脑损伤”,限定文献语种为中文。资料选择:共检索到相关文献500余条,选择与以下内容相关文章:①神经干细胞在脑缺血模型大鼠脑内的存活、迁移、分化,以及对颅脑损伤模型大鼠运动功能的影响。②神经干细胞在临床上的应用效果,包括对脑卒中患者的日常生活活动能力、神经功能的影响。③神经干细胞移植在临床上应用尚存在的问题。资料提炼:将初步筛选的文章进一步查找全文,排除综述和重复文献,纳入43条进行综述。其中论述脑卒中和颅脑损伤后主要的功能障碍问题的有14条,神经干细胞移植治疗脑卒中和脑损伤的动物实验文章25条,临床应用文章4条。资料综合:神经干细胞移植后可在脑缺血模型大鼠脑内存活、迁移、分化,从而使受损的脑组织得到修复。同时神经干细胞移植可改善脑损伤动物的运动和感觉功能。在临床方面,可改善大面积脑梗死患者的神经功能,以及脑卒中患者的日常生活活动能力。但目前神经干细胞移植治疗脑卒中和脑损伤尚主要处于动物实验阶段,且最终神经干细胞移植主要是通过分泌神经营养因子,增加有利于受损的脑神经细胞恢复的蛋白表达起作用,还是以其替代作用起主要作用尚不肯定。结论:应用神经干细胞移植在动物实验和临床中取得了一定正性效果,但仍存在问题,有待进一步解决。OBJECTIVE: To introduce the experiment and clinical effect of neural stem cell transplantation in the treatment of stroke and craniocerebral injury, and explore the clinical problems of neural stem cell transplantation. DATA SOURCES: The PUBMED database was undertaken to identify the articles on neural stem cell transplantation in the treatment of stroke and craniocerebral injury published from 2000 to 2007 with the key words of "neural stem cell, stroke,brain damage" in English. At the same time, Wanfang database was searched for relevant articles published from 2000 to 2007 with the same key words in Chinese. STUDY SELECTION: Totally 500 articles were retrieved, and relevant articles as follows were selected: ①survival, migration and differentiation of neural stem cells in rat models of cerebral ischemia, and the effect on motor function of craniocerebral injury rat models, ②applied effect of neural stem cells in clinic, including the effects on activities of daily living and neural function in stroke patients, and ③there were applied problems of neural stem cell transplantation in clinic. DATA EXTRACTION: The fuU-texts of articles in primary selection were searched, and articles of reviews and repetition were excluded. Totally 43 articles were involved for review. There were 14 articles on main functional disturbance after stroke and craniocerebral injury, 25 articles on animal experiment of neural stem cell transplantation for stroke and brain injury, and 4 articles on clinical application. DATA SYNTHESIS: Neural stem cells after transplantation could survive, migrate and differentiate in cerebral ischemia rat models to repair the damaged brain tissues. Simultaneously, neural stem cell transplantation could improve the motor and sensory function of animals with brain injury. In clinic, it could ameliorate the neural function of a large area cerebral infarction patients and activities of daily living in stroke patients.
