外伤性脱位前牙再植临床观察和相关因素分析24例

摘　　要：目的探讨脱位前牙再植的效果和相关影响因素。方法24例（36个前牙）完全性脱位行再植手术的病人,其中12例发生于学校,学校老师携带前来就诊3例,家属携带就诊20例,就诊时限0.5-10h,保存方式：冷水或盐水保存5例,口含保存5例,干置4例,脱位后弃置返回寻找保存于盐水10例。常规植入治疗,随访1-3.5年。结果20例存活,4例因牙根广泛吸收被拔除,成功率为83.3%。四种保存方式最终因牙根广泛吸收而拔除者中冷水保存1例,干置1例,弃置＋盐水2例。就诊时间在2.5h以下,除冷水保存有1例拔除外,均存活保留。结论脱位前牙再植的成功与就诊时间和保存方式有明显关系,而患者家属和学校教师的口腔保健治疗意识是影响这两个因素的主要原因。Objective To investigate the clinical effect and related dements of luxated teeth treated with replantation. Methods ： 24 cases（include 36 anterior teeth）of anterior teeth luxated treated with replantation. 12 cases of those happened in schools. But only3 of them came to see doctors with teachers and 20 cases came to see doctors with family members. The examination time was varied from 0.5 to 10 hours. For the storage 5 were stored in cold water or salty water. 5 were stored in mouths. 4 were left dry. And 10 were restored in salty water after doctors＇ demand. Results 4 of all 24 cases wereextracted because of extensive root resorption. One occured in water storage condition ,one in dry and two in lost and got. All anterior teeth the time until repantation leas than 2.5h kept alive,except one which was kept under water. Conclusion The most preferable management for the anterior tooth is replantation with in 20 - 30min after injury or keeping in an appropriate storage medium until replantation. Accordingly, the main relatively factor is the reorganization to oral hygiene of parents and school nurse- teachers.
