激素敏感型单纯性肾病病毒基因反式激活及病毒反义寡核苷酸的阻遏作用

摘  要:目的从正反两方面探讨NF-κB介导的病毒基因反式激活在呼吸道病毒感染触发激素敏感型单纯性肾病(steroid responsive simple nephrotic syndrome,SRSNS)中的作用。方法采用电泳迁移率改变实验、RT-PCR和ELISA分别测定42例SRSNS(包括32例活动期和10例缓解期)、15例肾炎性肾病、15例继发性肾小球疾病、20例毛细支气管炎和12例正常儿童外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)核因子-κB(nuclear factor kappaB,NF-κB)活性、呼吸道病毒(包括呼吸道合胞病毒和流感病毒)基因表达和血浆病毒抗体;将呼吸道病毒反义寡核苷酸(antisense oligonucleotides,ASONs)转染至SRSNS活动期患儿PBMCs,再检测呼吸道病毒基因表达和NF-κB活性。结果与SRSNS缓解期和其他组比较,SRSNS活动期组NF-κB活性明显增加。NF-κB活性与呼吸道病毒检出阳性率呈正相关趋势。SRSNS活动期PBMCs中呼吸道病毒基因的表达能被其相应的ASONs所抑制,同时NF-κB活性也降低。结论NF-κB介导的病毒基因反式激活是呼吸道病毒触发SRSNS的重要机制。Objective: To study the role of viral transactivation of transcription mediated by nuclear factor kappa B ( NF - κB) in steroid responsive simple nephrotic syndrome (SRSNS). Methods: 42 children with SRSNS were selected, including the active stage group (n= 32) and the remissive stage group (n= 10). 15 nephritic nephrosis, 15 secondary glomemlar diseases, 20 bronchiolitis and 12 healthy children were served as control, electro - mobility shift assays, RT - PCR and ELISA were used to detect the activity of NF- κB, the gene expression of respiratory tract viruses (including respiratory syncytial virus and influenza virus) in peripheral blood mononuclear cells (PBMCs) and the plasma levels of anti - virus antibody, respectively. After transfection of respiratory tract virus antisense oligonucleotides ( ASONs) into PBMCs from SRSNS at the active stage, the gene expression of respiratory tract viruses and the activity of NF - κB were measured through RT - PCR and electro - mobility shift assays, respectively. Results: Compared with that from SRSNS at the remissive stage and other groups, the activity of NF - κB in SRSNS at the active stage was much higher statistically. And there was a positive linear correlation trend between the activity of NF - κB and the gene expression of respiratory tract viruses in SRSNS at the active stage. After transfection of viral ASONs into PBMCs from SRSNS at the active stage, the gene expression of respiratory tract viruses and the activity of NF- κB were inhibited. Conclusion. Viral transactivation of transcription mediated by NF - κB may contributes to the pathogenesis of SRSNS triggered by respiratory tract viruses.
