摘 要:目的:探讨肝移植术后肝动脉缺血对肝内胆管上皮细胞自分泌VEGF-A、B、C及其受体(VEGFR)-1、2、3的影响;方法:对行常规改良Gao's肝移植术后的大鼠,分别实施二次手术:结扎肝动脉(HAL组);肝动脉微球注射并肝动脉结扎(MS+HAL组);对照组(SO组):肝动脉不做缺血处理。术后3、7d处死动物,取肝脏组织进行免疫组化和RT-PCR检测。结果:(1)VEGFR-1的蛋白表达在各组中或未见表达,或表达极弱。术后3、7dHAL组及MS+HAL组的的VEGF-A、B、C及VEGFR-2、3各项免疫组化阳性胆管上皮细胞的计数值均明显低于SO组,P<0.05。(2)各组均有VEGF-A、B、CmRNA及VEG-FR-1、2、3mRNA的表达,各组间无明显差异(P>0.05)。结论:肝移植术后肝动脉缺血时,肝内胆管上皮细胞自分泌并过度表达VEGF及其受体,从而促进胆管上皮细胞的代偿性增生。Objectives: To study the effect of autocrine VEGF of intrahepatic cholangiocytes after liver transplantation with ischemia of hepatic artery of rat. Methods: Rat orthotopic liver transplantation(OLT) was performed as described by Gao with modifications. Three days after OLT, the second procedure was performed and the groups were as foilowed: hepatic artery ligation (HAL group); injected microsphere through hepatic artery and hepatic artery ligation (MS+HAL group); sharm operation (SO group) without performing hepatic artery ligation. Twelve rats were included in each groups, and 6 rats were sacrificed at 3 and 7 days post-operation. Liver specimens were collected. Immunohistochemistry for VEGF-A, B, C and their receptors VEGFR-1, 2, 3 of cholangiocytes were performed. The expression of VEGF-A, B, C mRNA and their receptors VEGFR-1, 2, 3 mRNA in cholangiocytes were evaluated by RT-PCR. Results: The expression of VEGF-A, B, C and VEGFR-2, 3 of intrahepatic cholangiocytes were significantly lower in HAL and MS+HAL groups than in SO groups (P〈0.05) at 3 and 7 days post-operation, but VEGFR-1 was not expressed. In RT-PCR, there were expressions of VEGF mRNA and VEGFR-1, 2,3, but without significant differences among the groups (P〉0.05). Conclusion: Intrahepatic cholangiocytes of transplanted livers with ischemia of hepatic artery autocrined and over-expressed messenger RNA of VEGF-A, B, C and their receptors VEGFR-1, 2, 3, and intrahepatic cholangiocytes autocrined and over-expressed VEGF-A, B, C and VEGFR-2, 3, but did not express VEGFR-1. Over-expression of vascular endothelial growth factor might be the stimulus of vascular endothelial proliferation, which might consequently stimulate the growth of cholangiocytes of liver graft with ischemia of hepatic artery of rat.
