唐氏综合征筛查系统筛查效率的分析研究

摘　　要：[目的]探讨我市唐氏综合征筛查系统的筛查效率。[方法]抽孕妇全血2.0ml,取血清,使用BECK-MAN COULTER Access化学发光免疫分析系统,对孕妇血清中的AFP、β-HCG进行检测,结合孕妇年龄、孕周等相关因素,估算胎儿患DS和ONTD风险的概率。[结果]共筛查5333例孕妇,初筛出DS高危孕妇365例,初筛阳性率6.84%,365例中有4例DS孕妇,4968例DS初筛低危孕妇中有2例DS儿,检出率为66.7%。在5333例孕妇中,初筛出ONTD高危孕妇196例,初筛阳性率3.68%,196例ONTD初筛高危孕妇中异常妊娠孕妇13例,占6.33%,其中筛出4例ONTD儿,5137例ONTD低危孕妇中异常妊娠孕妇5例,占0.097%,其中无ONTD儿,ONTD检出率为100%。[结论]该套筛查系统DS检出率为66.7%,与国内报道的60%～70%和Wald等报道的60%的阳性检出率基本一致。ONTD检出率为100%,优于ThiagarjahS报道的75%～90%阳性检出率,说明DS筛查系统是可靠和有效的。[Objectives]To explore the efficiency of Down＇s Sydrome screening lab-system in our city.[Methods]AFP,β-HCG in the serum of pregnant woman were measured using full-automatic Access Immunoassay Analysis system of Beckman Coulter,which were combined with relevant factors such as pregnant woman＇s age and gestational weeks to estimate the probability of fetus suffering from DS and ONTD.[Results]From 5 333 pregnant women,365 cases with high DS risk were sift out in preliminary screening,and the positive rate was 6.84%. There were 4 DS in the 365 preener women and 2 DS in 4 968 pregnant women without high DS risk in preliminary screening, the examination rate was 66.7%. In 5 333 pregnant women, 196 cases with high ONTD risk were sift out in reliminary screening, and the positive rate was 3.68%. From 196 ONTD positive pregnant women, there were 13 abnormal pregnancy, which accounted for 6.33%, including 40NTD. From 5137 non-high ONTD risk pregnant woman, there were 5 abnormal pregnancy, accounting for 0.097%, in which there was no ONTD, and the ONTD examination rate was 100%. [Conclusions] In this system, the DS examination rate is 66.7%, in accord with the rate of 60%-70% reported by our country and Walds. The ONTD examination rate is 100%, better than 75%- 90% reported by Thlagarjah S. It has fully proved that the DS screening system is rehableand effective.
