摘 要:目的检测家兔失神经支配腮腺导管改道术后唾液的成分,并与家兔正常腮腺唾液和泪液成分作比较,探讨失神经支配腮腺导管改道术后唾液替代泪液的可行性。方法日本大耳白兔40只,随机分为4组,每组10只。第一组摘除左侧泪腺、哈氏腺、瞬膜并用50%三氯醋酸烧灼睑球结膜制成眼干症模型,将左侧腮腺导管转移到左下睑穹隆。第二组在制作眼干症模型基础上,切断左侧鼓室神经,再将左侧腮腺导管转移到左下睑穹隆。第三组将左侧腮腺导管内置管固定于皮肤。第四组不作任何手术。第一、二组术后第3个月取唾液-泪液,第三组术后取正常唾液,第四组取正常泪液。采用常规生化检查方法测定所有唾液及泪液标本的成分并进行比较。测定项目包括淀粉酶、尿素、葡萄糖、钠、钾、氯、白蛋白、总蛋白、甘油三酯、溶菌酶、渗透压以及pH值。结果家兔失神经支配腮腺导管改道术后唾液钠、氯含量、pH值与正常泪液无明显差异,其甘油三酯含量与正常泪液和腮腺唾液无明显差异。其余成分均有差异,但失神经支配腮腺导管改道术后唾液成分更接近于泪液。结论家兔失神经支配腮腺导管改道术后唾液成分与正常泪液、腮腺唾液的成分有差异,但其介于正常泪液与腮腺唾液之间,表明腮腺失神经支配后唾液成分发生改变,用失神经支配腮腺导管改道术后唾液替代正常泪液是可行的。Objective To compare the quality of saliva secreted by the denervated parotid gland with duct transposition with saliva from normal parotid gland and tear in the rabbits, and to investigate the feasibility of using saliva as a substitute of tear. Methods Forty rabbits were divided into four groups ramdomly with 10 rabbits in each group. The parotid duct transposition was performed after establishment of a rabbit model of dry eye in the first group. In the second group tympanic neurectomy was underwent in addition to the same surgical procedure as in the first group. The parotid duct was cannulated in the third group. There was no treatment in the fourth group. The "saliva - tear", saliva and tear were obtained and analyzed. Results Sodium, chlorine content and pH of tear were not different significantly from those of saliva secreted by the denervated parotid gland with duct transposition. There was no difference among triglyceride compositions of tear, saliva and saliva - tear. The other compositions of tear, saliva and saliva - tear were varied to some extent. Conclnsian The compositions of tear were different from those of saliva - tear and saliva. But the compositions of saliva secreted by the denervated parotid gland with duct transposition were very close to those of normal tear. The saliva secreted by the denervated parotid gland with duct transposition can be used as a substitute of tear.
