Twinblock矫治安氏Ⅱ~1类错畸形硬组织改变的研究

摘　　要：目的:探讨Twinblock矫治器矫治替牙期或恒牙早期安氏Ⅱ1错畸形硬组织学的改变。方法:选择19例下颌后缩、上颌基本正常的安氏Ⅱ1类错畸形患者,年龄10～13岁,采用Twinblock矫治器进行治疗,对治疗前后头颅侧位片进行测量,并对治疗前后测量结果进行配对t检验。结果:患者治疗前后面型明显改善,SNB、LI-MP、Go-Co、Go-Gn显著性增大(P<0.01);SNA、ANB、UI-SN、OJ显著性减小(P<0.01);MP-FH改变无统计学意义(P>0.05)。结论:Twinblock矫治器对安氏Ⅱ1类错具有矫形作用,可以诱导下颌功能性前移,促进下颌生长,抑制上颌生长,同时使得上下颌牙及牙槽骨发生改变,从而协调上下颌关系,改善面型。Objective To investigate hard tissue changes caused by Class Ⅱ division 1 malocclusion correction in the patients treated with Twin-block appliance during mixed and early permanent dentition. Methods 19 cases with mandibularretrusion aged 10-13 years old were treated with Twinblock appliance. 9 measurements of cephalometrics before and after the treatment of Twinblock were carried out. Pared t test was used. Results All cases with madibular retrusion were corrected. After the treatment SNB,LI-MP, Go-Co and Go-Gn were increased （P〈0.01）.SNA,ANB,Ul-SN and OJ were reduced （P〈0.01）. The change of MP-FH had no statistically significant （P〉0.05）. Conclusion Twin-block functional appliance possesses the orthopedic effect on Angle ClassⅡ division 1 malocclusion. When repositioning the mandible forward and improving the growth of mandib, it creates the force to limit maxillary growth forward as well. Simultaneously it causes the changes of dento-alveolar.AII these aspects contribute to a harmonious relationship of upper and lower jaws and a better profile.
